复杂性颧上颌骨骨折治疗体会

摘　　要：目的:通过24例复杂性颧上颌骨骨折的临床治疗回顾,对其诊断、治疗方法及并发症进性分析总结。方法:24例复杂性颧上颌骨骨折患者三维CT检查,确定骨折部位,均采用切开复位坚强内固定术,其中18例患者术后两天行颌间牵引治疗2～4周,术后三月复查。结果:24例患者术后均恢复良好的咬合关系和开口度、20例面形恢复满意、4例面形改善明显、7例出现麻木、5例额纹变浅、3例视力明显下降。结论:手术治疗是复杂性颧上颌骨骨折治疗的首选方法,辅助颌间牵引对咬合关系的恢复意义明显。对其伤情判定、复位固定方法、手术并发症、视力障碍等问题需进一步研究。Objective：Through review clinical therapy of 24 patients with complieated malar-maxillary frneture,we analyzed an summarized its diagnosis,therapy and complications.Methods：After checked up by three diamensions CT,24 patients with complicated mate maxillary fracture were made sure the fracture site,cnfireed open reduetions and intornal fixations,18 patients acepted intenmarliary traction 2 days later after the operations,lasting for the periods about 2-4 weeks.All the patients were follow-up 3 months after operations,Results：All the patients got favourable occluding relation and opening degree .In face shape,satisfactory effects were achieved in 20 patients and obvious improvement in 4 patients But there were numbness in 7 patients,shallow forehead wrinking in 5 patients and obvious decrease of vision in 3 patients.Conclusious：operation for complieated malar-maxillary fracture was the first choice.Assisted intermaxillary traction had important role to occluding relation.But how to judgement the traumatic condition,way to reposition and fixture,complications,decrease of vision,et al,need further research.
