城市社区老年人护理意愿及临终关怀需求调查分析

摘　　要：目的了解城市社区老年人对护理和临终关怀的需求情况，为开展城市社区卫生服务和临终关怀提供参考。方法自行设计问卷对开封市社区297名60～90岁老年人的健康状况、护理服务需求及临终关怀需求进行调查。结果204人（68．7％）患有慢性疾病，以高血压（116人，39．1％）、冠心病（74人，24．9％）多见；护理方式需求选择就近医疗168人（56．6％）、上门护理66人（22．2％）；生活不能自理时选择在家中204人（68．7％）、住养老院78人（26．3％）；81人（27．3％）对临终关怀有所了解；对于临终关怀地点，189人（63．6％）选择在家中、45人（15．2％）拒绝回答此问题。结论社区老年人的护理意愿多选择就近和在家中，临终关怀认知较差；需加强医疗基础设施建设和死亡教育，以满足社区老年人就医、养老需求，帮助他们树立正确的生死观。Objective To understand the demands of nursing and hospice care in aged population of city communities, and to provide reference for development of the community health service and hospice care. Methods Self-designed questionnaires about health conditions, demands of nursing and hospice care were filled out by 297 elderly people aged strengthened 60-90 in a certain community of Kaifeng city. Results Strictly, 204（68.70%） subjects suffered from chronic diseases, in which 116 subjects had hypertension and 74 had coronary heart disease; 168（56.6%）of them chose health care near residence, 66（22.2%） of them selected home visit; when they could not take care of themselves, 204（68.7 %）of them wanted to stay at home, and 78 （26.3%） of them chose nursing home;81 （27.3 %） of them knew little about palliative care; as for the places for palliative care, 189 （63.6 %） of them chose home, 45 （15.2 % ） of them refused to answer this question. Conclusion The elderly residents in city tend to choose nursing care near or at home, and they lack knowledge about palliative care. Infrastructure and health education on death should be strengthened to satisfy their medical care and pension demands and guide them to establish correct concept of life and death.
