摘  要:目的通过与自体髂骨进行比较,评价rhBMP-2/20%β-TCP/PDLLA复合物的成骨能力。方法采用浇铸盐析技术制备三维多孔20%β-TCP/PDLLA、20%β-TCP/PDLLA并与rhBMP-2复合。4个月龄健康雄性新西兰兔24只,体重2.8~3.4kg,随机分为A、B两组各12只,根据Boden介绍的方法制作L4、5横突间植骨融合模型,A组植人rhBMP-2/20%β-TCP/PDLLA复合物,B组植入自体髂骨块。动物按术后处死时间(3周、6周、12周),行大体观察、X线检查观察新骨形成,人工骨降解情况,及早期组织学表现。结果复合人工骨与自体髂骨移植均能达到良好的脊柱融合,12周时,复合人工骨不仅成骨早、新骨形成量大,而且在新骨形成,改塑及的同时载体材料逐渐降解,术后12周可形成较为成熟的骨小梁结构,所形成融合块较大、强度高。但自体髂骨移至术后12周其所形成的新骨结构与其相比,尚需进一步塑形及完善。结论20%B-TCP/PDL-LA是很有希望的骨移植替代材料。其结构、降解产生微环境均有利于rhBMP-2发挥作用具有强的诱骨活性及良好的骨性融合能力,其融合力强于自体髂骨移植。从而有望成为脊柱外科所需的新型人工骨移植材料。中、远期疗效的深入研究有待进一步完善。Objective By comparison with autologous iliac bone, evaluate rhBMP- 2/20% β-TCP/PDLLA complex osteogenic potential. Methods Preparation of casting salting using three - dimensional porous 20% β- TCP/ PDLLA ,With rhBMP - 2 composite. 4 - month - old healthy male New Zealand rabbits 28 ,weighing 2.8 - 3.4 kg,were randomly divided into A, B groups of 14. According to the method described in Boden production IA ,5 transverse inter- body fusion model. A group of implanted rhBMP - 2/20% β- TCP/PDLLA complex, B group implanted autogenous iliac bone block. Animals were killed according to postoperative time (3 weeks,6 weeks, 12 weeks) , for gross observation, X - ray observation of the formation of new bone, artificial bone degradation, and early histological manifestations. Results Both composite artificial bone and autologous iliac bone graft can both achieve good spinal fusion,12 weeks, the composite artificial bone into the bone is not only early,large amount of new bone formation, but also in new bone formation, reshaping, and at the same time the gradual degradation of carrier materials and after 12 weeks may be the formation of a more mature trabecular bone structure, formed by fusion blocks of large, high strength. However, autolo- gous iliac bone 12 weeks after they moved to the formation of new bone structure and its comparison requires further shaping and perfect. Conclusions 20% B - TCP / PDLLA is a promising bone graft substitutes. Its structure, degrada- tion resulting micro - environment are conducive to the role of rhBMP - 2 induced bone has a strong activity and good bony fusion capabilities, its integration and strong in the autogenous iliac bone graft. Spinal surgery which is expected to be required for the new artificial bone graft. In - depth study of long - term efficacy remains to be further improved .
