摘 要:目的观察特发性黄斑前膜(IERM)的三维光学相干断层扫描(3D-OCT)图像的形态特征,探讨3D-OCT对IERM的诊断价值。方法回顾性分析2009年1月至5月间确诊为IERM的患者35例(40只眼),行视力、裂隙灯、眼底彩照和OCT检查。OCT检查采用TOPCON3D-OCT1000MARKII,扫描速度27000A扫描/s,扫描面积6.0mm×6.0mm,扫描模式128×512,分析中心凹厚度、中心凹形态及感光细胞(IS/OS)情况。结果IERM的3D-OCT图像表现为视网膜表面薄厚不一的高反光带,绝大多数伴程度不同的中心凹厚度增加。在40只眼中,弥漫性水肿或囊肿20只眼,假性黄斑裂孔3只眼,板层裂孔3只眼,玻璃体黄斑牵拉4只眼,中心凹形态正常者10只眼。结论3D-OCT可提供更多的IERM中心凹形态的信息,在IERM确诊方面有很高的灵敏性。Objective To observe the three-dimensional optical coherence tomography (3D-OCT) characteristic of idiopathic epiretinal membrane (IERM) and investigate the clinical apphcation of 3D-OCT. Methods The 40 eyes ( 35 patients ) with IERM were included in this study. All the patients had undergone examination of visual acuity, sht lamp, ocular fundus and 3D-OCT with the speed of 27000 A scan/s, area of 6.0mm ×6. 0mm, and mode of 128 ×512. Results IERM was visible on 3D-OCT as high reflect tissue. In most of the fovea, the thickness increased. Macular morphological changes were categorized into types of macular edema ( ME ) or foveal cyst ( FC ), lamellar maeular hole ( LMH ), macular pseudohole ( MPH ), vitreomaeular traction ( VMT ) and normal according to the findings of OCT. 20 of 40 eyes were found with ME or FC, 3 eyes with LMH, 3 eyes with MPH, 4 eyes with YMT and 10 eyes were normal in studied patients. Conclusion 3D-OCT can objectively display morphological changes in IERM. It is a useful tool to measure these lesions.
