57例垂体瘤患者视野分析

摘　　要：目的分析垂体瘤患者的视野特征。方法回顾性研究57例垂体瘤患者的视野报告及MRI影像资料。结果早期垂体微腺瘤患者,视野大多正常。随着肿瘤增大,典型的视野缺损表现为垂直性颞侧、双颞侧偏盲。视野缺损与视交叉自身解剖位置密切相关。视交叉位置正常者,视野缺损呈典型改变。视交叉前置或后置者,视野表现为同向偏盲、视敏度下降、中心暗点或交界性暗点。结论双颞侧偏盲是垂体瘤视交叉受压上抬典型的视野改变。视野缺损与视交叉解剖位置相关,前置型、后置型的视交叉者,视野缺损趋于多样性。Objective To analysis the feature of visual fields with pituitary adenomas. Methods Visual fields and MRIs of 57 patients who had suffered from pituitary adenoma and treated in No. 2 hospital of Soochow university were retrio- spectively observational study. Results In the early stage,the visual fields of pituitary microadenomas is normal. With the developing of adenomas, the most common type of visual fields defects is vertical hemianopia and bilateral temporal hemianopia and unilatenal hemianopia. The type of visual field defect is connected with location of the optic chiasm itself. If the location of the optic chiasm is normal, the visual fields defects is typical. If the location of the optic chiasm is before or after, the visual field presents nomonymous hemianopia or nasal hemianopia or central scotoma or junction scotoma. Severly,it is monocular or bilateral optic astrophy. Conclusion Bilateral temporal hemianopia is typical visual fields defects in patients with impairment of optic chiasm with pituitary adenomas. The type of visual fields defects is connected with the location of optic chiasm itself. The feature of visual field presents irregular and various if the location of optic chiasm is before or after.
