弥漫大B细胞淋巴瘤CD5表达与CD10、bcl-6、MUM1表达及预后的关系

摘　　要：目的:检查CD5在淋巴结内弥漫大B细胞淋巴瘤(DLBCL)中的表达情况,了解CD5表达与肿瘤细胞中CD10、bcl-6、MUM1表达的关系,以及上述分子标记对肿瘤的临床病理特征和预后的影响。方法:HE染色观察淋巴结内DLBCL的组织学特征,免疫组织化学染色检测CD5、CD10、bcl-6、MUM1和Ki-67表达。结果:39例DLBCL中25例表达CD5,DLBCL的CD5阳性和阴性组的CD10,bcl-6,MUM1、Ki-67的表达、年龄、性别、体能指数及生存状况无显著性差异,但两组的临床分期有显著性差异。CD10(-)bcl-6(-)MUM1阳性组与阴性组之间生存状况无显著性差异,CD10和(或)bcl-6表达组与MUM1表达组及Ki67表达>20%与≤20%病例之间生存状况无明显差异。结论:DLBCL中CD5表达与CD10、bcl-6、MUM1的表达无明显相关,但与肿瘤的临床分期有关,CD5阴性病例的分期较低;CD5表达对DLBCL的其它临床特征和生存状况无明显影响,CD10、bcl-6、MUM1表达对DLBCL的生存状况有一定影响。Objective ： To detect the expression of CD5 in nodal diffuse large B cell lymphoma （ DL- BCL） and understand the correlation between CD5 expression and the expression of CD10,bcl-6, and MUM1 in DLBCL as well as its effect on the clinicopathological features and prognosis of DLBCL. Methods： Pathological specimens of 39 cases of DLBCL were collected and their histopatholigical features were observed with HE stained sections. Some immunophenotypes of DLBCL （ CDS, CD10, bcl- 6, and MUM1, etc. ） and Ki-67 expression were detected by using immunohistochemistry staining. Result： Expression of CD5 was detected in 25 DLBCL cases out of 39. There was no statistical difference of expression of CD10, bcl-6, MUM1, or Ki-67 between CD5-postive （ CD5 ^＋ ） and -negative （CD5 ^- ） groups, neither the survival status or clinicopathological features such as age, sex and so on were statistically different between the 2 groups. While there were indeed statistical differences of staging between the two groups. The differences of survival status between CD10^- , bcl-6^- , and MUM1^- postive and -negative groups were not significant. There was no obvious difference of survival between CD10/bcl-6-postive and MUMl-postive groups, or between Ki-67≥20% and 〈20% groups. Conclusions： The expression of CD5 is not related to the expression of CD10, bcl-6, MUM1 in DLBCL,but is related to the staging of DLBCL. The stages are lower in CDS-negative cases than those in CD5- postive cases. CD5 expression is not associated with other clinical features or survival status of DLBCL. The expressions of CD10, bcl-6, MUM1 affect the prognosis of DLBCL.
