髁状突囊内骨折摘除术5例临床观察

摘　　要：目的:探讨髁突囊内骨折摘除术的诊断、手术治疗和预后情况。方法:对5例患者经耳前入路行髁突囊内摘除术,术后行临床及影像学检查。结果:术后随访6～24个月,全部患者疗效满意,无神经损伤和关节症状。1例出现轻度错畸形,并伴有中度开口偏斜。X线显示,患侧髁突改建后外形大致正常或略低于正常髁突,CT显示其中1例患侧髁突出现吸收现象,咬合关系不佳。结论:严格掌握髁突囊内骨折摘除术的适应证,规范的操作,可以获得良好的临床疗效。Objective To investigate diagnosis, surgery method and prognosis of intracapsular condylar fractures with removing the fragments. Methods A preauricular approach was used for exposure, and removel in 5 cases, Postoperative radiologic, and clinical follow-ups were performed. Results All patients were satisfied with the clinical results, slight malocclusion and lateral deviation during mouth opening exceeding 2mm existed in 1 cases. Radiologic follow-up revealed that the injured condylar processes had become roughly proper or a little less proper. Moderate resorption was seen in 1 case by CT scanning, but without any obvious function disturbances. Conclusion Once we master the strict indication and normal the operation,we can acquire satisfactory result from treating intracapsular condylar fractures by removing the fragments.
