希罗达联合草酸铂治疗晚期胃癌的临床观察

摘　　要：目的：观察草酸铂联合希罗达治疗晚期胃癌的疗效及安全性。方法：73例晚期胃癌患者随机分为两组,治疗组采用草酸铂85 mg/m^2静脉滴注2 h,第1天,希罗达1000 mg/m^2口服,第1～14天,每3周为一周期,对照组草酸铂85 mg/m^2静脉滴注3 h,第1天,CF200 mg/m^2静脉滴注2 h,第1～5天,5-Fu 300 mg/m^2静脉点滴5 h,第1～5天,每3周为一个周期。每例患者至少接受4个疗程,按照WHO实体瘤近期客观疗效评定标准进行评价。结果：治疗组（希罗达组）有效率50%,对照组有效率46.8%,无统计学意义（P〉0.05）两组疗效相当。两组重度毒性反应,治疗组（希罗达组）少于对照组差异有统计学意义（P〈0.05）,其中重度白细胞减少,两组对比差异无统计学意义,而重度腹泻,便秘,重度口腔炎及重度恶心、呕吐,两组对比有统计学意义（P〈0.05）。外周神经毒性为L-OHP的突出毒副反应,呈可逆性。结论：L-OHP联合希罗达治疗晚期胃癌疗效肯定,毒副反应轻。有利于提高化疗患者的生存质量,患者乐于接受治疗,依从性好。Objective： To evaluate the efficacy and safety of Xeloda combined with oxaliplatin in the treatment of patients with advanced gastric cancer. Methods： 56 patients with advanced gastric cancer were randomly divided into two groups. The treatment group were administrated with oxaliplatin plus Xeloda （oxaliplatin 85 mg/m^2 infused intravenously for 3h day 1 ; Xeloda taken orally 1000 mg/m^2 day 1 to 14, repeated 21 days）. The control group were administrated with oxaliplatin,CF and 5-Fu（oxaliplatin 85 mg/m2 infused intravenously for 3 h day 1, CF200 mg/m2 and 5- Fu 300 mg/m^2 infused intravenously day 1 to 5 ,repeated 21 days）o Each patient should complete at least four cycles and was evaluated according to WHO solid tumor efficacy of the recent assessment criteria. Results： The effective rate were 50% and 46.8% in the treatment group and control group respectively. But no statistical significance were deeteeted （P 〉 0. 05）. The side effects of severe toxicity in treatment group （Xeloda group） were less than that of the control group significantly （P 〈 0.05）. But there is no significant difference between the two groups in severe leukopenia. While the moderate and severe diarrhea, constipation, severe stomatitis and severe nausea, vomiting in the two groups was statistically significant （P 〈 0.05）. Peripheral nerve toxicity of LOHP prominent toxicity was reversible. Conclusion： L-OHP combined Xeloda in the treatment of advanced gastric cancer is effective and has mild side effects. It helps the patients improve their quality of life ; they are willing to accept the treatment.
