急性梗阻性左半结肠癌Ⅰ期切除吻合59例治疗分析

摘　　要：目的:探讨左半结肠癌并急性梗阻行I期切除吻合术的安全性和可行性。方法:回顾性分析59例急性梗阻性左半结肠癌手术治疗患者的临床资料。结果:结肠脾曲癌11例,降结肠癌34例,乙状结肠癌14例,行I期结肠癌切除吻合术53例,Hartmann手术3例,单纯近端结肠造瘘3例。无死亡病例,吻合口瘘2例。结论:正确掌握急性梗阻性左半结肠癌手术时机、灌洗方法,合理选择手术方式,做好围手术期处理,尽量选择I期切除吻合术是安全可行的。Objective： To explore the safety and feasibility of one - stage resection and primary anastomosis for acute obstruction of left hemicolonic carcinoma. Methods： The clinical data of 59 patients with left colon carcinoma combined with acute bowel obstruction were retrospectively analyzed. Results： There were 11 cases with colon - spleen area carcinoma,34 cases with decending colon carcinoma, 14 cases with colon sigmoideum carcinoma. There were 53 patients with colon carcinoma primary resection and anastomosis surgery,3 patients with Hartmann surgery,3 patients with simple proximate colon stoma surgery . There were 2 cases with stoma fistula, without decease cases. Conclusion： It was important to handle the surgery time , surgery modus and irrigation method of acute obstruction left hemicolon carcinoma. It was safety and feasible to chose primary resection and anastomosis.
