吗啡控缓释片及芬太尼贴剂治疗癌痛的临床观察

摘　　要：目的:观察吗啡类控缓释片与芬太尼贴剂治疗中重度癌性疼痛的疗效及不良反应。方法:回顾分析应用吗啡类控缓释片和采用芬太尼贴剂治疗中重度疼痛327例患者临床疗效和药物不良反应的资料。结果:应用吗啡类控缓释片281例中,完全缓解161例(57.3%),明显缓解70例(24.9%),中度缓解12例(4.3%),轻度缓解19例(6.8%),总缓解率93.3%;采用芬太尼贴剂治疗46例中,完全缓解24例(52.2%),明显缓解11例(23.9%),中度缓解7例(15.2%),轻度缓解2例(4.3%),总缓解率95.6%;应用吗啡类药物不良反应主要有:嗜睡(昏睡)42例(14.9%),恶心/呕吐87例(30.9%),便秘55例(19.6%),排尿困难29例(10.3%),中断治疗7例(严重恶心、呕吐6例,昏睡1例),完全无不良反应70例(24.9%)。采用芬太尼贴剂治疗不良反应主要有:嗜睡6例(15.1%),恶心呕吐12例(26.0%),便秘3例(7.1%),排尿困难4例(8.7%),皮肤过敏3例(6.5%)无中断治疗,完全无不良反应14例(30.4%)。两种治疗对病人的生存质量的提高均有显著性差异(P<0.05)。结论:吗啡控缓释片及芬太尼贴剂,是目前在临床上用以控制中重度癌痛的有效药物。Objective：To observe the curative effect and side effect of opiumism and Fentanyl pastes for the middle and serious cancerous pain. Methods： To analyze the clinical curative effect and side effect of opiumism and Fentanyl pastes in 327 middle and serious pain patients. Results：In 281 patients using opiumism ,completely alleviates 161 （57.3%） ,obviously alleviates 70 （24.9%） ,moderate alleviates 12 （4.3%） ,mildly alleviates 19（6.8% ）, overall alleviating rate 93.3% ;In using Fentanyl pastes 46 patients completely alleviates 24 （52.2%） ,obviously alleviates 11 （23.9%） ,moderate alleviates 7 （ 15.2% ） ,mildly alleviates 2 （4.3%） ,overall alleviating rate95.6% ; Main side effect of opiumism ：drowsiness 42 （ 14.9% ） ,disgust/vomits 87 （30.9%） ,constipation 55 （ 19.6% ） ,difficult urinating 29 （ 10.3% ）, severance treatment 7 （ seriously disgust/vomits 6 and drowsiness 1 ）, no side effect 70 （ 24.9% ）. Main side effect of using Fentanyl pastes ： drowsiness 6 （ 15.1% ）, disgust/vomits 12 （26.0%）, constipation 3 （7.1% ）, difficult urinating 4 （8.7 % ）, skin allergy 3 （6.5 % ）, no severance treatment, completely non side effect 14 （30.4%）. Conclusion：The controlled release morphine and Fentanyl pastes are effective medicine in clinical to control the middle and serious pain patients.
