目血康联合清开灵、血栓通治疗视网膜静脉阻塞的临床观察

摘　　要：目的探讨中药目血康胶囊联合清开灵、血栓通注射液治疗视网膜静脉阻塞的临床疗效。方法将视网膜静脉阻塞93例(104眼)患者分成两组,治疗组62例70眼,用目血康胶囊每次3粒,每天3次口服,清开灵40 mL、血栓通注射液6～10mL每天1次静脉滴注;对照组31例34眼,口服复方丹参滴丸,尿激酶2万单位每天1次静脉滴注。2组均15 d为1个疗程,休息5 d后,再继续用1个疗程。观察并比较2组用药前后视网膜出血吸收、视力、血液流变学等情况。结果治疗组总有效67眼(占95.7%),痊愈38眼(占54.2%);对照组总有效24眼(占70.6%),痊愈11眼(占32.4%),2组比较差异均有统计学意义(P均<0.05)。治疗组治疗前后血液流变学,除全血中切黏度外,其余全血高切黏度、全血低切黏度、血浆黏度、纤维蛋白原、红细胞聚集指数均明显降低(P均<0.05)。2组治疗后纤维蛋白原、全血高切黏度比较,治疗组均明显低于对照组(P均<0.05)。结论目血康联合清开灵、血栓通治疗视网膜静脉阻塞,能够促进视网膜出血吸收,提高视力、改善患者血液流变学指标,疗效确切。Objective To evaluate the clinical efficacy of Muxuekang capsule combined with injections of Qingkailing and Xueshuantong for retinal vein occlusion（RVO）.Methods Ninety-three RVO patients（104 eyes） were divided into treatment group（62 cases,70 eyes） and control group（31 cases,34 eyes）.Treatment group received Muxuekang capsule,3 pills one time and 3 times one day,intravenous injection of 40 mL Qingkailing and 6-10 mL Xueshuantong,once daily;Control group received compound Danshen Root drop pills orally and intravenous injection of 20×103 U urokinase,once daily.For the both two groups,15 days were a course of treatment,and for another course of treatment after having a rest for days.The retinal hemorrhage absorption,visual acuity,and hemorheological index were evaluated and compared before and after treatment in two groups.Results There were 67 eyes improved（95.7%） and 38 eyes cured（54.2%） in treatment group,and 24 eyes improved（70.6%） and 11 eyes cured（32.4%） in control group.There was significant difference between two groups（both P〈0.05）.Compared with those before treatment,the hemorheological index such as whole blood high viscosity at high shear,whole blood viscosity at low shear,plasma viscosity,fibrinogen and erythrocyte aggregation index decreased significantly（all P〈0.05）,but not the whole blood viscosity at middle shear.Fibrinogen and whole blood viscosity at high shear was lower in treatment group compared with control group after treatment（both P〈0.05）.Conclusion Muxuekang capsule combined with injections of Qingkailing and Xueshuantong can promote retinal hemorrhage absorption,improve visual acuity and hemorheological index,and with exact effects in the treatment of retinal vein occlusion.
