摘 要:目的 探讨脑胶质瘤基质金属蛋白酶2(MMP-2)表达与肿瘤病理分级、磁共振成像(MRI)特征的关系。方法31例脑胶质瘤病例术前均行MRI平扫及增强扫描,术后病理证实,并分为低级别组(Ⅰ、Ⅱ级)20例和高级别组(Ⅲ、Ⅳ级)11例。免疫组化染色观察MMP-2表达情况。结果高级别胶质瘤组MMP-2表达、瘤周水肿指数(edemaindex,EI)、强化程度(enhancepercentage,EP)、肿瘤最大直径显著高于低级别胶质瘤组(t值分别为8.200,4.405,6.286及5.026,P<0.05)。MMP~2表达与其对应的EI、EP、肿瘤最大直径成正相关(r值分别为0.480,0.754及0.700,P〈O.05)。MMP-2在边界清晰和不清晰组间表达无显著性差异(t=-0.103,P〉0.05),信号均匀组和不均匀组间表达存在显著性差异(t=2.848,P〈0.05)。结论MMP~2表达强度与脑胶质瘤MRI征象,包括瘤周水肿、强化程度、肿瘤最大直径、信号均匀与否密切相关。Objective To investigate the levels of matrixmetalloproteinase-2(MMP-2) in different grades of human gliomas and its relationships with characteristics of MRI. Methods 31 cases of gliomas confirmed by pathology underwent plain and contrast -enhanced MR imaging. The lesions were divided into two groups:low grade group( grade Ⅰ and Ⅱ) in 20 cases and high grade group (grade Ⅲ and Ⅳ )in 11 cases. The MMP-2 expression levels were evaluated by immunohistochemical staining. Results The expression levels of MMP-2, edema index around the tumors( EI), enhanced percentage of the tumors(EP) and the tumor's maximum diameter were significantly higher or obvious in the high grade group than that in the low grade group [t=8. 200 , 4. 405 , 6. 286 and 5. 026, respectively,P〈0.05). The expression of MMP-2 were found significantly correlated to the EI, EP and the maximum diameter of the tumors(r=0. 480,0. 754 and 0. 700, respectively, P〈0.05). The expression levels of MMP-2 were not significantly different between the tumors with and without definite borders [ t =0. 103, P〉0.05 ), while, there was significantly different between tumors with homogeneous and unhomogeneous signal intensity(t=2. 848, P〈0.05). Conclusion MMP-2 expression is of closely relationship with MR imaging features of gliomas,including EI,EP, maximum diameter and signal heterogeneity of the tumors.
