摘  要:目的探讨水通道蛋白-1(AQP-1)在肝细胞癌(HCC)、肝内胆管癌(ICC)和转移性腺癌(MAC)中的表达特征及其与临床病理学的关系。方法应用免疫组化EnVision Plus法对100例HCC、50例ICC和30例MAC组织中AQP-1、Hep Par-1、CKl9和pCEA的表达进行检测,并对其与肿瘤临床病理特征的关系进行统计学分析。结果ICC组AQP-1表达率为88%(44/50),明显高于HCC组(7/100,P<0.01)和MAC组(6/30,P<0.05);随着ICC组织学分化程度的降低,AQP-1表达明显下降(P<0.05),ICC患者淋巴结转移AQP-1的表达率明显低于未转移者(P<0.01)。结论AQP-1可能是胆管细胞分化及其肿瘤的良好标记物,结合Hep Par-1、CKl9和pCEA等可有助于肝肿瘤的鉴别诊断。Objective To study the correlation of morphological characteristics and aquaporin-1 (AQP-1) expression with elinieopathologic features in bepatocellular carcinoma (HCC), intrahepatic cholangiocarcinoma (ICC) and metastatic adenocarcinomas (MAC) in order to evaluate the possible diagnostic implication of this marker. Methods One himdred HCC, 50 ICC and 30 MAC cases with neighboring noncancerous tissues samples were stained with AQP-1, Hep Par-1, CK19 and pCEA by immunohistoehemical method. Staining patterns of AQP-1 were counted, and then statistically analyzed in conjunction with the clinieopathological features of liver cancers. Results AQP-1 expression was found in 88 % (44/50) of all ICCs compared with 7%(7/100) ofHCC (P 〈 0.001) and with 20% (6/30) of MAC (P = 0.037). Poorly differentiated histology of ICC was significantly more prevalent in the negative AQP-1 group than in the high AQP-1 group ( P = 0.012). Low or negative AQP-1 expression was associated with positive lymph node metastasis in ICC ( P = 0.007). Conclusion AQP-1 seems to be a highly selective marker for differentiated cholangiocytes and cholangiocareinomas, and it may be very belpful in the differential diagnosis of liver tumors when combined with Hep Par-1, CK19 and pCEA.
