超长眼轴高度近视白内障超声乳化手术疗效观察

摘　　要：目的观察超长眼轴高度近视白内障超声乳化手术的疗效。方法对37例50眼超长眼轴高度近视白内障患者进行超声乳化手术。术中透明角膜切口下充分水分离白内障核后,用劈核勾把晶体核勾起至虹膜面进行劈核,再用45%～50%的能量,80～100mmHg负压进行乳化吸除,抛光后囊膜,观察术后视力、术后反应、并发症及用药情况。结果本组病例术后两天矫正视力≥0.5眼者25眼占50%,术后1个月矫正视力0.05以上者45眼,占90%;术后角膜内皮轻度水肿5眼,占10%;后囊混浊6眼,占12%。结论观察超长眼轴高度近视白内障超声乳化手术脱盲率达90%,角膜水肿发生率低,达到良好手术效果,且视力不良者与眼底病变、斜视、弱视有关。Objective To observe the effect of phacoemulsification for super-long axial length of high myopia with cataract. Methods 37 cases （50 eyes） of super-long axial length of high myopia with cataract were treated with phacoemulsification. In the operation, cataract nucleus was water separated through clear corneal incision, the lens nucleus was pulled out to iris face and chopped, and then the cataract nucleus was emulsified with 45-50% energy and 80-100mmhg negative pressure; polished posterior capsular. Postoperative visual acuity, postoperative reaction, complications and situation of medication were observed. Results The corrected visual acuity ≥0.5 was in 25 eyes （50%） at two days after operation; the corrected visual acuity over 0.05 was in 45 eyes （90%） in one month after operation, corneal edema in 5 eyes （10%） and posterior capsule opacification in 6 eyes （12%）. Conclusion The out of blindness rate is 90% after phacoemulsification for super-long axial length of high myopia with cataract; the incidence of corneal edema is lower,can obtain good surgical effect. The poor vision is related to fundus diseases, strabismus and amblyopia.
