预防宫颈癌根治术后尿潴留措施的新探讨

摘　　要：目的对宫颈癌根治术后尿潴留的防治进行探讨。方法对在我院首次接受宫颈癌根治术的120例患者,随机分为实验组和对照组各60例。实验组采用术后第3天开始进行膀胱冲洗,第5天进行提肛训练和个体排尿训练,拔管时预留膀胱冲洗液,拔管后高锰酸钾溶液坐浴等综合措施;对照组按常规方法留置尿管。两组患者术后留置尿管时间均为8～10d,拔除尿管6～7h后自行排尿2～3次后测残余尿量。结果拔除尿管后实验组发生尿潴留3例,再置管率为5.0%;对照组发生尿潴留19例,再置管率为31.7%。两组患者在排尿顺畅程度、残余尿量、尿潴留的发生率差异有统计学意义。结论对宫颈癌根治术后,运用综合干预措施,可促进膀胱功能恢复,预防尿潴留发生。Objective To explore the protective measure of urinary retention after the radical hysterectomy of cervical carcinoma. Methods We choose 120 cases with cervical carcinoma that were treated by the first radical hysterectomy and divide randondy into 2 groups of 60 cases of experiment and control. The experimental group are treated by the bladder wash out 3 days after operation and the function training of the levator ani musculature and the individual training of emiction 5 days after operation, we remain some liquid of bladder washout and demibain in the liquor potassic permanganate after pull out the urethral catheter. The control group are treated by the routine method of remaining the urethral catheter. The two groups time of remaining the urethral catheter both are 8-10 days. Then we measure the remaining quantity of the urine in the bladder 2-3 after pulling out the urethral catheter. Results The experimental group has 3 cases of urinary retention and the rate 5% of urethral catheter again. But the control group has 13 cases of urinary retention and the rate 31.7% of urethral catheter again. There＇s a significant difference between the experimental group and the control group in the degree of smooth emiction and the remaining quantity of urine and the rate of uroschesis. Conclusion We can adopt the general methods on the patients after radical hysterectomy of cervical carcinoma to improve the convalescence of the bladder＇s function and decrease the rate of uroschesis.
