摘  要:目的探讨肿瘤药敏试验在食管癌化疗选药中的作用。方法将43例患者的食管癌组织进行体外培养,以临床常用的7种化疗药物加以干预,MTT法检测化疗药物对癌细胞的抑制率。结果食管癌对丝裂霉素、氟尿嘧啶、多西他赛、泽菲、环磷酰胺、健择、顺铂中度敏感,7种化疗药物对食管癌细胞的抑制率无显著性差异(P=0.69)。对食管低分化鳞癌细胞的抑制率显著高于高分化者(P=0.025),DDP对食管中分化鳞癌细胞的抑制率显著高于高分化者(P=0.017)。食管上段、中段、下段癌细胞对以上7种化疗药物的敏感性无显著性差异(P>0.05)。结论DDP、对食管癌细胞分化程度低者抑制作用较强。初次化疗的食管癌患者对多种化疗药物均较为敏感,但各药的抑制率无显著性差异,故可先使用价格便宜的普通药物,当病情出现反复时再更换化疗方案,以避免耐药,从而提高疗效,并节省费用。Objective To characterize cancer drug sensitivity' s effect on chemotherapy selection for oesophagus cancer. Methods We cultured tissue of 43 oesophagus cancer pations in vitro,intervered with 7 chemotherapy drugs used commonly in clinic, determined inhibitory rate of chemotherapy drugs to cancer cells by MTT. Results Oesophagus cancer was medium sensitive to mitomyein, 5 - fluorouraeil, doeetaxel, Zefei, cyclophosphamide, gemzar and cisplatin. There was no significant difference in inhibitory rate of 7 chemotherapy drugs to oesophagus cancer ceils( P = 0.69 ). Poorly differentiated oesophagus cancer cells were inhibited more than well differentiated oesophagus cancer cells by 5 - fluorouraeil( P = 0. 025 ). Medium differentiated oesophagus cancer cells were inhibited more than well differentiated oesophagus cancer cells by cisplatin( P = 0. 017). Oesophagus cancer cells of up, medium and down piece were of no significant difference in sensitivity to the 7 chemotherapy drugs( P 〉 0.05). Conclusion Cisplatin and 5 - fluorouraeil inhibit poorly differentiated oesophagus cancer cells more. Oesophagus cancer cells are much sensitive to several kinds of chemotherapy drugs that are the first time to accept the chemotherapy. There is no significant difference in inhibitory rate of the chemotherapy drugs. We can use cheap and common chemotherapy drugs at first. When the illness worsen,it is time to alter drugs to avoid drug resistance ,improve treatment effect and reduce treatment cost.
