腔内热灌注治疗颌骨巨大角化囊性瘤的临床研究

摘　　要：【目的】探讨腔内热灌注治疗牙源性角化囊性瘤的治疗效果。【方法】本研究选用1999-2007年在我院就诊的牙源性角化囊性瘤患者共63例,对其中21例颌骨病灶实施骨腔内刮除及70℃生理盐水热灌注治疗,36例选用袋形术,6例采用截骨植骨术。术后进行临床、组织学及影像学表现观察其疗效。【结果】腔内热灌注治疗后使颌骨的完整性得到保存,经1-2年以上观察,均达到临床治愈效果,无1例复发,与截骨植骨组无统计学差别。术后颌骨成骨速度及复发率明显高于袋形组。【结论】通过颌骨骨腔内热灌注治疗,较好地解决了牙源性角化囊性瘤术后复发问题,达到保存颌骨完整性和功能性的目的。【Objective】To evaluate the effect of hyperthermia used for keratocystic odontogenic tumour, to avoid fracture and bone graft and preserve the integrality and function of jaw.【Methods】A total of 63 patients with mandibular keratocystic odontogenic tumour were treated operatively.21 patients with disease of jaw were treated more than two times by marsupialization,curettage and hyperthermia of 70℃ normal saline.6 patients were healed by resection.36 patients were treated with marsupialization.Therapeutic effects were observed by clinical manifestation,histology and imageology.【Results】The integrality of jaw was preserved effectively post-operatively.No recurrence was noticed during 1～2 years followed up.Good clinical purpose was showed.【Conclusions】Intraluminal hyperthermia may preserve the integrality and function of jaw in case of odontogenic tumor and withno recurrence.
