腹水标本癌胚抗原mRNA检测诊断恶性腹水

摘　　要：目的分析良、恶性腹水癌胚抗原mRNA(CEA mRNA)检测差异,探讨应用CEA mRNA鉴别诊断良、恶性腹水的价值。方法对38例恶性腹水和32例良性腹水检测CEA mRNA和CEA,比较各组此2个指标结果的差异。结果CEAmRNA和CEA的阳性率分别为:恶性腹水78.9%和52.6%,良性腹水9.4%和6.2%。恶性腹水组CEA mRNA和CEA阳性率均高于良性腹水组,差异有显著性(P<0.05)。恶性腹水检测CEA mRNA阳性率明显高于CEA阳性率,差异有显著性(P<0.05)。结论恶性腹水CEA mRNA的阳性率显著高于CEA,对鉴别诊断恶性腹水比CEA更有帮助,但对肝癌性腹水不敏感。Objective To analyze the difference in carcinoembryonic antigen mRNA（CEA mRNA）expression in patients with benign and malignant ascites and to study the value of CEA mRNA in differential diagnosis of benign and malignant ascites.Methods The expression of CEA mRNA and CEA level were detected in 38 cases of benign ascites and 32 cases of malignant ascites.The differences were compared in patients of these two groups.Results Positive rates of CEA mRNA and CEA were 78.9% and 52.6% respectively in malignant group and 9.4% and 6.2% respectively in benign group.The positive rate of CEA mRNA expression in malignant ascites was obviously different from that of benign ascites（P0.05）.Conclusion The positive rate of CEA mRNA expression was obviously higher than CEA level in patients with malignant ascites.So detection of CEA mRNA expression is useful in diagnosis and differential diagnosis of malignant ascites,but it is not sensitive in diagnosis and differential diagnosis of ascites caused by hepatic carcinoma.
