摘 要:目的:探讨粘附分子CD44v6和基质金属蛋白酶MMP-9在宫颈癌中的表达及其与宫颈癌浸润转移的关系。方法:采用免疫组织化学S-P法检测CD44v6、MMP-9在10例正常宫颈组织(NCE)、18例宫颈上皮内瘤变(CIN)和37例宫颈浸润癌(ICC)中的表达情况,分析其表达与宫颈癌临床病理特征之间的关系以及CD44v6、MMP-9二者的相关性。结果:CD44v6与MMP-9的表达在ICC中最高,CIN中次之,NCE中最低;CD44v6、MMP-9的表达在淋巴结转移组明显高于无淋巴结转移组(P<0.O5),间质浸润深组明显高于间质浸润浅组(P<0.O5);CD44v6与MMP-9两者在宫颈癌中的表达呈正相关,其共同高表达者发生间质浸润深层和淋巴结转移的几率明显高于仅其中之一阳性表达或皆为阴性表达者。结论:CD44v6、MMP-9阳性表达率随正常宫颈组织-宫颈上皮内瘤变-宫颈癌的转化而增加;CD44v6、MMP-9的过表达可能促进宫颈癌浸润和淋巴结转移,且二者在宫颈癌浸润转移过程中相互关联、协同作用。Objective: To explore the expression levels of CD44 v6 and matrix metalloproteinase - 9 ( MMP - 9) in patients with cervical carcinoma and the relationship between CD44v6 and MMP- 9 and invasion and metastasis of cervical carcinoma. Methods: The expression levels of CD44 v6 and MMP- 9 in 10 cases with normal cervical tissues, 18 cases with cervical intraepithelial neoplasia (CIN) and 37 cases with invasive cervical carcinoma were detected by immunohistochemical S -P method, the relationship between CD^v6 and MMP - 9 and clinical pathologic characteristics of cervical carcinoma was analysed, and the correlation between CD44 v6 and MMP -9 was explored. Results: The expression levels of CD44 V6 and MMP -9 were the highest in invasive Cervical carcinoma, higher in CIN and the low- est in normal cervical tissues ; the expression levels of CD44 V6 and MMP - 9 in patients with lymphatic metastasis were significantly higher than those in patients without lymphatic metastasis (P 〈 0. 05 ), and the expression levels of CD44 V6 and MMP -9 in deep stromal involve- ment group were significantly higher than those in superficial stromal involvement group ( P 〈 0.05 ) ; there was a positive correlation between CD44v6 and MMP -9 in cervical carcinoma, the probability of deep stromal involvement and lymphatic metastasis in patients with co- expression of CD44v6 and MMP -9 was higher than that in patients with positive CD44 v6 or positive MMP -9 or negative CD44 V6 and MMP - 9. Conclusion: The positive rates of CD44 V6 and MMP -9 increase gradually with the progress of normal cervical tissues -CIN -invasive cervical carcinoma. The overexpression of CD44 V6 and MMP-9 are related to invasion of cervical carcinoma and lymphatic metastasis, and their functions are synergistic and interdependent.
