前纵隔淋巴瘤的CT特征与病理对照初探

摘　　要：目的探讨前纵隔不同病理类型淋巴瘤的CT特征。资料与方法回顾性分析20例不同病理类型的前纵隔淋巴瘤患者的CT征象。结果(1)部位:20例CT均表现为前纵隔肿块,前上区2例,前下区1例,前中区17例;4例肿块偏一侧生长,16例肿块向双侧生长;18例为多结节融合性肿块,其中17例肿块伴坏死,2例为密实肿块。(2)病理类型:霍奇金淋巴瘤1例,T细胞型淋巴瘤4例,大B细胞型淋巴瘤13例(弥漫性5例、间变性7例、富于T细胞性1例),滤泡细胞性淋巴瘤1例,边缘区B细胞型淋巴瘤1例。(3)继发改变:11例侵犯上腔静脉、5例侵犯右肺、5例侵犯心包并心包积液、8例合并胸腔积液、1例侵犯胸骨。结论前纵隔不同病理类型的淋巴瘤CT表现也有所不同,仔细分析有助于定性并与前纵隔其他恶性肿瘤进行鉴别。Objective To investigate the CT features of anterior mediastinal lymphoma in different pathological types. Materials and Methods 20 cases with anterior mediastinal lymphoma confirmed by pathology were retrospective analyzed. The CT features were reviewed. Results （1） location： All the tumors located at anterior mediastinum, among them, the tumors located at superior anterior mediastinum in 2 cases, inferior anterior mediastinum in 1 case, upper median area in 17 cases. 4 cases projected toward one-side and 16 cases toward double-side. There were soft tissue nodus around tumor in 18 cases, 17 cases had necrosis,2 cases were density mass. （2） Pathology type：l case was Hodgkin Lymphoma, 4 cases were cytotype T Lymphoma, 13 cases were cytotype big B Lymphoma （diffused 5 cases, anaplasia 7 cases, 1 case alive with cellular type T）, 1 case was follicular cells, 1 case was marginal zone of cytotype big B Lymphoma. （3） Secondary changes ： Superior vena cava were involved in 11 cases, right lung were involved in 5, neighborhood blood vessel embed by tumors in 5, pleural effusion could be seen in 8, combined with hydropeficardium in 5, sternum was involved in 1. Conclusion The anterior mediastinal lymphomas have different CT features in different pathological types, which can be helpful to distinguish from other anterior mediastinal malignant tumors.
