摘　　要：目的:研究Maspin基因在大肠癌中的表达情况,探讨其在大肠癌发生、发展、转移过程中的作用及其与预后的关系。方法:应用免疫组织化学技术(PV二步法)分别对大肠癌原发灶、腺瘤、正常大肠黏膜、淋巴结转移灶、肝转移灶进行Maspin的检测,应用统计学方法比较各种组织中的表达情况。结果:Maspin在正常大肠黏膜、腺瘤、大肠癌原发灶中的表达依次下调;Maspin在伴有肝转移的大肠癌原发灶中的表达低于不伴有肝转移的大肠癌原发灶中的表达(P<0.05);Maspin在各种不同大体病理类型中的表达不同,随着恶性程度的提高而下调(P<0.05)。Maspin(+)组患者生存时间长于maspin(-)组。结论:Maspin作为一种抑癌基因,其表达下调在大肠癌的发生、发展及转移的过程中可能起到一定的作用。Maspin可能成为判断大肠癌有无肝转移的预测指标,为大肠癌肝转移的早期诊断提供线索。判断大肠癌的预后。Objective To investigate maspin expression in colorectal carcinoma and it＇s role in tumorgenesis, progression and metastasis of colorectal carcinoma, and assess the relationship between maspin expression and prognosis of colorectal carcinoma. Methods Maspin expression in colorectal carcinoma, eoloreetal polypous adenoma tissues, normal colorectal tissue, lymph node metastasis and liver metastasis was investigated by PV immunohistochemical assay. Data of maspin expression in each tissue was analyzed using statistical methods. Results Maspin expression showed a significant decrease from normal eolorectal tissue to adenoma and adenoearcinoma. Maspin expression in coloreetal carcinoma with liver metastasis was lower than that without liver metastasis （P 〈 0.05）. Maspin expression in different pathological type was down-regulated with the increasing of malignant degree （P 〈 0.05 ）. The patients with maspin expression survive longer than those without maspin expression. Conclusions Down regulation of maspin expression plays an important role in tumorgenesis, progression and metastasis of colorectal carcinoma. Maspin expression will be a prediction index for liver metastasis of eolorectal carcinoma and a index for prognosis analysis of patients with colorectal carcinoma.
