鼻内镜技术在鼻腔鼻窦内翻性乳头状瘤切除术中的应用

摘　　要：目的：探讨鼻内镜技术在鼻腔鼻窦内翻性乳头状瘤切除术中的应用价值。方法：共在内镜下切除鼻腔鼻窦内翻性乳头状瘤26例,其Ⅱ级25例,均为鼻腔外侧壁侵犯部分上颌窦及前后组筛窦。1例为Ⅳ级病变,有前期手术史,鼻腔后段广泛菜花样肿物,坠至鼻咽部并侵犯腭部骨质穿孔,约有1cm直径菜花样肿物突出至口腔。结果：术后随访12个月-3年,其中Ⅱ级25例均无复发,术腔上皮化良好,1例Ⅳ级病变局部有复发,微波热凝后无复发,遗留软硬腭交界处穿孔,直径约2cm,19个月后随访肿瘤无复发,做二期修复腭部穿孔。结论：经鼻内镜切除鼻腔、鼻窦内翻性乳头状瘤可以较好地保存鼻腔的重要结构,尤其是对于Ⅰ级和Ⅱ级病变,符合微创手术原则,手术历时短,术中出血少、是彻底切除肿瘤的较好方法,对于病变较广泛的可以采用联合进路彻底切除肿瘤。Objective： To study the value of resection of nasal sinus inverted papilloma under endoscope. Method： 26 patients with inverted papilloma in nasal cavity and paranasal sinuses in stage I - IV were treated successfully by endoscopic telescopes. Result： All patients were followed up for 12 months to 3 years after surgery, no recurrences occurred in the following up period. Conclusion： Resection of nasal sinus inverted papilloma under endoscope gains minimally invasive effects, and also less destructive to important nasal structures in the nasal cavity. Endoscopic telescopes is an effective method in treating of the inverted papilloma.
