乳头旁憩室与胰胆疾病ERCP检查的关系

摘　　要：目的：探讨不同类型乳头旁憩室与胰胆疾病之间的关系以及对内镜下逆行胰胆管造影（ＥＲＣＰ）插管成功率的影响。方法：回顾分析本院１９９４～１９９６年进行ＥＲＣＰ检查的１４２２例患者（１２７例有乳头旁憩室，１２９５例无憩室）的内镜下表现、插管情况及最终诊断。结果：有憩室组总插管显影１２６例，胰胆管显影１００例，选择性插管失败３例。有憩室组插管后显影的１２６例中有胆胰系统疾病者９６例（７６．２％），无憩室组阳性发现３５５例（２７．４％），两组比较有显著差异（Ｐ＜０．００１）；其中胆管结石和胆管癌的发现率明显高于无憩室组，不同乳头旁憩室组间各种疾病发现率无差异。结论：憩室的存在可能和某些胰胆疾病的发生有关，而不同类型乳头旁憩室间胰胆疾病的发生无显著差异。Objective: To investigate the relation between the types of diverticula around the papilla and the diseases of bile duct and pancreas, and whether the diverticulum may influence the success of cannulation for endoscopic retrograde cholangiopancreatography (ERCP). Methods: To review retrospectively the endoscopic findings, cannulation and ERCP diagnosis in 127 cases of peripapillary diverticula among 1 422 ERCP cases from 1994 to 1996. Results: One hundred and twenty six cases of diverticula succeeded in cannulation and 100 cases of double ducts succeeded. Selective cannulation failed in 3 cases. Ninety six cases (76.2%) among 126 cannulated cases had pancreaticobiliary diseases, the rate was significantly higher than that in those of no diverticula cases (355/1 295, 27.4%)examined in the same period (P<0.001), especially between the diseases of bile duct stones and bile duct neoplasms. There was no difference in different types of peripapillary diverticulum and the incidence of pancreaticobiliary disease. Conclusion: The incidences of some pancreaticobiliary diseases in those patients with peripapillary diverticula are higher than those with no diverticula, and are not related to different type of peripapillary diverticulum. The peripapillary diverticulum has no influence on the success in cannulating by means of some methods.
