摘  要:目的观察人血白蛋白与纳络酮联合治疗急性大面积脑梗死的疗效及安全性。方法2007年7月至2009年5月本院36例急性大面积脑梗死患者,随机分为治疗组和对照组各18例。治疗组应用人血白蛋白与纳络酮联合治疗;对照组应用常规脱水降颅压及活血化瘀、抗血小板聚集和奥拉西坦治疗。评价两组治疗前、治疗后7,14,21,28d的神经功能缺损评分和日常生活能力评分。结果治疗后14d治疗组神经功能缺损评分和日常生活能力改善均较对照组明显(P<0.05),有效率高于对照组,死亡率明显降低;治疗后28d,治疗组和对照组神经功能缺损评分的有效率分别为44.4%和27.7%(P<0.01),日常生活能力评分的有效率分别为61%和27.7%(P<0.01)。不增加不良反应。结论人血白蛋白与纳络酮联合治疗能有效改善急性大面积脑梗死患者神经功能缺损和日常生活能力,未见明显不良反应。Objective To observe the therapeutic efficacy and safety of the combined treatment of Albumin Prepared from Human Plasma and Naloxone on acute massive hemispheric infarction. Methods The cases of massive hemispheric infarction treated in our department from July, 2007 to May, 2009 were divided randomly into treatment group ( n = 18 ) and control group ( n = 18 ). The former was treated with Albumin Prepared from Human Plasma and Naloxone, while the latter was treated with Oxiracetam and conventional therapy. The scores of the neurological deficits (ESS) and activities of daily living(ADL) were recorded before the treatment and 7, 14, 21,28 d after the treatment. Results The scores of the neurological deficits (ESS) and activities of daily living ( ADL ) were improved in both groups. But the improvement in the treatment group was greater than that in control group ( P 〈 0. 05 ) 14 d of treatment . The treatment group had a significantly higher effective rate and lower death rate . After 28 d of treatment , the effective rates of the neurological deficits (ESS) in treatment group and control group were 44.4% and27. 7% respectively (P 〈 0. 01 ) ,while those of ADL were 61% and27. 7% respectively ( P 〈 0. 01 ). The adverse reactions showed no significant difference in two group. Conclusion The combined therapy of acute massive hemispheric infarction with Albumin Prepared from Human Plasma and Naloxone can improve effectively neurological deficits and activities of daily living without conspicuous adverse reactions.
