摘　　要：目的观察LASIK手术切削光区大小和瞳孔大小对手术后暗视下对比敏感度的影响。方法LASIK手术72例(144眼),采用德国Oculus公司MesotestⅡb对比敏感度仪,检测LASIK术前及术后3月暗视下有或无眩光两种情况的对比敏感度的变化,比较通过与未通过对比度等级第3和第7级(contrast level 1:2.7)患眼手术切削光区及瞳孔大小。结果术后3月无眩光情况下MESOTEST对比敏感度阈值各等级未通过的例数均比术前增加,但仅第4级有统计学差异。有眩光情况下MESOTEST对比敏感度阈值各等级未通过的例数均比术前增加,第6、7、8级有统计学差异。术后3月有和无眩光情况下通过与未通过MESOTEST对比敏感度阈值等级测试3级和7级(contrast level 1:2.7)患者的平均Orbscan瞳孔大小差异无统计学意义;而未通过患眼平均切削光区小于通过者,差异有统计学意义。结论LASIK术后早期暗视下对比敏感度比术前降低,LASIK手术切削光区大小是影响术后夜间视觉质量的重要因素,小光区对暗视下对比敏感度的影响较为明显,尤其是有眩光情况下更为显著。Objective To analyze the influence of pupil and optical zone diameter on contrast sensitivity under mesopic luminance condition in LASIK patients. Methods Contrast sensitivity with and without glare was determined in 72 patients （ 144 eyes ） who had undergone LASIK in Changhai hospital from September to December in 2007, tests were performed using the Mesotest Ⅱ b （ Oculus, Wetzlar, Germany） under mesopic conditions before surgery and at 3 months postoperatively. Pupil and optical zone diameter were compared between the patients who ableor unable to discriminate the mesoplc contrast limits of 1：2.7 （grade 3 and 7; contrast level 1 ： 2.7 ）. Results Compared to preoperatively, on the 3rd month under Mesopic condition without glare the proportion of patients unable to discriminate the mesopie contrast limits of grades rose, but not significantly except grade 4 （contrast level 1：2.0）, but increased obviously with glare at grade 6 （ contrast level 1 ： 5 ）, grade 7 （contrast level 1：2.7 ） and grade 8 （contrast level 1：2.0 ）, there are statistical significance ; On the 3rd month, under mesopic condition with and without glare, there was no difference between the Orbscarl pupils of patients who able or unable to discriminate the mesopic contrast limits of 1：2.7, but the means opticalzone di- ameter of patients who unable to discriminate it was smaller then patients who able to, there are statistical significance. Conclusion Mesopic contrast sensitivity was worse in LASIK patients at early stage, under mesopic conditions, the effect of small optical zone on contrast sensitivity after LASIK is obvious, especially with glare.
