摘 要:目的探讨谷胱甘肽转硫酶(GST)PI基因型与溃疡性结肠炎(UC)及散发性大肠癌(SCRAC)易感性的关系。方法采用聚合酶链反应技术(PCR)检测354例(UC96例、SCRAC118、健康人群140例)的GSTP1基因型,比较该基因型在各组之间的分布差异。结果GSTP1基因突变型(Ⅱe/Val+VaL/VaL)频率在UC组和SCRAC组中明显增高,与对照组比较,差异有统计学意义(48.96%比31.43%,P〈0.01,0R=0.478,95%CI:0.280~0.817;45.77%比31.43%,P〈0.05,OR=0.534,95%CI:0.327~0.903);分层分析发现VaL/VaL基因型在远端UC中的分布频率高于广泛结肠UC(χ2=5.682,P〈0.05,OR:3.594,95%CI:1.218~10.608);VaL/VaL基因型与SCRAC病变部位无关(P〉0.05),但在DukesC期和低分化SCRAC中分布频率明显增高(P〈0.05)。结论GSTP1基因型与湖北汉族人群UC和SCRAC的易感性相关。Objective To investigate the association between the susceptibility to ulcerative colitis (UC) and sporadic colorectal adenocarcinoma (SCRAC) and the genotypes of glutathione S-Transferase P1 ( GSTPI ). Methods PCR was used to study the GSTP1 genotypes, and the discrepancies of GSTP1 genotype frequencies among 96 UC patients, 118 SCRAC patients and 140 healthy controls were analyzed byx2 test. Results The frequency of VaL/VaL genotype was higher in the UC patients and the SCRAC patients than in the controls (48.96% vs 31.43% ,P 〈 0.01, OR = 0.478,95% CI:0. 280-0. 817 ; 45.77% vs 31.43% ,P 〈 0.05, OR = 0.534,95% CI:0. 327-0.903 ). Stratification analysis revealed that the frequency of VaL/VaL genotype was higher in patients with distal colon UC than in those with extensive colon involvement (χ2 = 6.665, P 〈 0.05, OR = 0.229,95 % CI:0. 070-0. 745 ). There was no association between the frequency of VaL/VaL genotype and the location of SCRAC ( P 〉 0.05 ). However, the frequency of VaL/VaL genotype was significantly associated with colorectal adenocarcinoma of Dukes C classification (χ2 = 7. 380, P 〈 0.01, OR =0.347,95% CI:0. 160-0.753 ) and poor differentiation (χ2 = 00.213,P〈0.01 , OR =0. 270 ,95% CI:0. 124-0.590),respectively. Conclusion The GSTP1 genotype is strongly correlated with the susceptibility to UC and SCRAC in Chinese Hans.
