摘 要:目的对口腔门诊外科操作过程中手套破损的发生进行风险评估。方法对口腔门诊外科手术中医师使用的3798只手套破损情况进行调查。将手术过程分为3个研究组:Ⅰ组:局部麻醉、抬高黏膜瓣过程;Ⅱ组:去除骨或牙根过程;Ⅲ组:软组织修复过程。结果3798只手套破损发生率和发现率分别为9.2%、13.1%;Ⅱ组、Ⅲ组的手套破损率明显高于Ⅰ组(P<0.05);在软组织修复过程中,左手手套的破损率明显高于右手手套的破损率(P<0.05);破损部位多发生于左手掌侧食指端。结论口腔外科医师应加强使用医用手套的防护意识,建议外科医师术中左手采用双层局部加强防护手套。Objective To assess the incidence of glove puncture during oral surgical procedures. Methods We collected and analyzed the punctures from a total of 3789 gloves in 252 surgical procedures from March to October 2008. Each surgical procedure was divided into three modules, namely, ( Ⅰ ) administration of the anesthesia and raising of mucoperiosteal flap, ( Ⅱ ) bone removal and delivery of tooth root, and, ( Ⅲ ) soft tissue repair. Ressults The puncture rate of gloves and detection rate of gloves puncture in 3789 gloves were 9.2% and 13.1% , respectively. The incidence rates of glove puncture were significantly higher in procedure Ⅰ ( P 〈 0.05 ) and proudure Ⅱ ( P 〈 0.05 ) than that in procedure Ⅲ. The incidence rates of glove puncture in the left hand and the right hand were 29.8% and 15.0% , respectively. Left hand (the nonworking hand) gloves in procedure Ⅲ showed higher incidence of puncture than right hand gloves ( P 〈 0.05 ) , especially in the index fingers. Conclusions To prevent cross infection, the medical staff should beware of glove puncture during oral surgical procedures. If is suggested the surgeon might wear double gloves in the nonworking hand during the surgical procedure.
