摘 要:目的探讨针刺治疗心肌缺血的神经源性机制,并观察针刺干预在缺血再灌注损伤心肌保护中的作用机制。方法将50只大鼠随机分为5组①空白组,②假手术组,③模型组,④高频电针组,⑤低频电针组。5组均麻醉后捆绑,全程记录心电图。空白组同期捆绑对照;假手术组开胸挂线不结扎;模型组、高频电针组和低频电针组开胸结扎大鼠左冠状动脉前降支,造成心肌缺血20min,再灌注40min。经以上处理后颈动脉取血,采用721分光光度计测定血清缺血修饰白蛋白(IMA)和免疫组织化学法测定下丘脑β3-肾上腺素受体(β3-ADR)。结果模型组大鼠心肌缺血再灌注后心电图STⅡ值明显高于空白组和假手术组,也高于低频电针组和高频电针组。模型组IMA含量、β3-ADR表达较空白组和假手术组明显升高,而低频电针组和高频电针组与之比较显著降低;高频电针组大鼠心肌缺血再灌注后IMA含量和β3-ADR表达都低于低频电针组。结论电针内关穴能减少心肌缺血再灌注损伤大鼠血液中IMA含量,降低下丘脑β3-ADR阳性表达。提示电针通过对神经传导通路上相关物质的影响,对心肌缺血再灌注伤害性神经信号传递过程中产生抑制作用,从而抑制应激反应的发生,起到保护心肌的作用。[ Objectives ] To probe the nervours original mechanism in treating myocardial ischemia by acupuncture, and observe the mechanism of action in protecting the MIRI cardiac muscle with acupuncture. [Methods] Fifty rats were randomly divided into five groups, such as blank control group, sham operated group, model group, lower frequency of electro-acupuncture group and higher frequency of electro-acupuncture group. All these five groups were bound in anesthesia and their ECG were recorded during the whole process. The blank group was bound as the control at the same time. The sham operated group was operated to open the thoracic cavity and was braid through the artery with no ligation; the latter three groups must be operated to open their thoracic cavity and ligate left descending coronary artery, to result in myocardial ischemia for 20 minutes, and reperfusion for 40 minutes following. After all manipulation above, we took carotid artery blood to measured IMA by spectrophotometer and impression of β3-ADR by immunohistochemistry method. [ Results ] The value of ST Ⅱ in ECG of model group after MI and reperfusion was super to the blank group and sham operated group obviously, moreover super to high frequency manipuhion group and low frequency manipulation group. The IMA content and β3-ADR impression of model groupwere increased significantly, compared with blank control group and sham operated group, but decreased in low frequency of electro-acupuncture and higher frequency of electro-acupuncture. The value of ST Ⅱ in ECG, IMA and β3-ADR of low frequency of electro-ncupuncture were lower than higher frequency of electro-acupuncture after MIRI. [ Conclusion] Electroacupuncting at Neiguan point can decrease the IMA content and the CGRP content of myocardial ischemia and reperfusion rats and degrade the value of ST Ⅱ in ECG, leading to innocent protect to the heart muscles.
