北京地区新生儿全血10种氨基酸含量参考值范围的调查研究

摘　　要：目的建立北京地区新生儿全血10种氨基酸含量的正常参考值范围，为临床诊治新生儿氨基酸代谢异常性疾病提供依据。方法采用串联质谱法检测北京地区313例正常新生儿的10种氨基酸水平，其中包括有苯丙氨酸（Phe），蛋氨酸（Met），异亮-亮氨酸（Leu—Ile），缬氨酸（Val），精氨酸（Arg），瓜氨酸（Cit），半胱氨酸（Cys），组氨酸（His），乌氨酸（Orn），酪氨酸（Tyr）。将所有新生儿根据性别不同分成男、女两组；根据胎龄分为足月儿组、早产儿组。分析比较不同性别和胎龄组之间的氨基酸水平是否存在差并，进而得到串联质谱检测新生儿干血滤纸片氨基酸参考范围。结果经过正态性检验，新生儿氨基酸水平呈偏态分布，因此，所有数据均经过对数转换后进行统计分析。经统计后发现不同性别新生儿组的氨基酸水平之间差异无统计学意义（P〉0．05）。不同胎龄的新生儿组部分氨基酸（Met，Phe，Tyr）水平差异有统计学意义（P〈0．05）。计算新生儿各种氨基酸的正常参考值范围。结论建立串联质谱法检测新生儿的各种氨基酸的正常参考值范围对于临床诊治和筛查氨基酸代谢异常性疾病十分重要。Objective To build the correct reference values of amino acids of newborns in Beijing and provide clinicians with laboratory evidence. Methods The levels of 10 amino acids on the dry blood spots of 313 healthy newborns in Beijing were detected by tandem mass spectrometry. 10 amino acids included Phe ,Met ,Leu-Ile ,Val ,Arg ,Cit ,Cys ,His ,Orn and Tyr. According to different genders,newborns were divided into male and female two groups ;according to conceptus age,the newborns were divided into premature newborns group and term newborns group. The levels of amino acids of different gender groups and conceptus age groups were compared,and the reference values of amino acids on the dry blood spots of newborns were calculated. Results According to the results of test of normality ,the levels of amino acids of newborns belong to skewness distribution. Transformations of normality were done by data of amino acids. There were not obvious significance of amino acids in different gender groups （P〉0. 05）. There were obvious significance of amino acids in different conceptus age groups （P〈0. 05）. The reference values of amino acids of children were calculated. Conclusion It is a very important job to provide clinicians with correct reference values of amino acids of newborns detected by tandem mass spectrometry.
