摘 要:目的:三阴性乳腺癌分为基底样和非基底样2种表型,其中基底样表型预后差。文中探讨基底样标记物细胞角蛋白5/6(CK5/6)和表皮生长因子受体(EGFR)在三阴性乳腺癌中的表达。方法:免疫组化(EnVision法)检测80例三阴性及25例非三阴性浸润性乳腺癌中CK5/6和EGFR蛋白的表达,运用荧光原位杂交(FISH)检测2例EGFR免疫组化可疑阳性者EGFR基因扩增情况。结果:组织学上肿瘤组织具有推进式的边缘,周围伴淋巴细胞浸润。瘤细胞呈合体细胞样,通常见坏死。所有病例显示为高级别癌,核分裂象易见。免疫组化示三阴性乳腺癌和非三阴性乳腺癌中CK5/6的表达率分别为72.5%和4.0%(P<0.05),EGFR表达率分别为61.3%和28.0%(P<0.05)。三阴性乳腺癌中CK5/6与EGFR的表达具有相关性(P<0.05)。2例经FISH检测见EGFR基因扩增。结论:大多数(>60%)三阴性乳腺癌具有基底细胞样表型同时表达EGFR。Objective: Triple-negative breast carcinoma can be classified into basal and non-basal phenotypes. The basal phenotype is associated with poor outcome. Our aim was to investigate the expression of basal Markers (CK5/6 and EGFR) in triple-negative invasive breast carcinoma. Methods: The immunohistochemical method was used to detect the expressions CK5/6 and EGFR in 80 cases of triple-negative and 25 cases of non-triple-negative breast carcinoma. The EGFR gene was tested by fluorescence in situ hybridization (FISH) in 2 cases, and the related literature was reviewed. Results: Histologically, the triple-negative cases revealed pushing margins of the tumors with peri-tumoural lymphocytic infiltrate, a syncytial growth pattern, and areas of necrosis. All the cases were shown to be high grade carcinoma with a very high mitotic index. The expression rates of CK5/6 were 72.5% and 4.0%, respectively, in the triple-negative and non-triple-negative cases (P 〈 0.05 ), and those of EGFR were 61.3% and 28.0% (P 〈0.05). There was a relationship between the expressions of CK5/6 and EGFR in triple-negative breast carcinoma. The FISH test of EGFR in the 2 eases showed gene amplification. Conclusion : Most eases ( 〉 60% ) of the triple-negative invasive breast carcinoma have the basal subtype with a high expression of EGFR, which suggestively responds to EGFR-targeted therapies.
