摘 要:目的:研究鼻咽黏膜上皮内淋巴细胞(mucosa intraepithelial lymphocyte,mIEL)在鼻咽癌时的变化,初步探讨局部上皮增生活跃程度对mIEL的影响。方法:(1)收集可见黏膜上皮的51例鼻咽慢性炎症(NPi)和90例鼻咽癌(NPC)石蜡标本。(2)在HE切片上计数黏膜上皮内淋巴细胞、癌巢内淋巴细胞,判定巢间淋巴细胞浸润程度。(3)对部分病例用CD3、CD4、CD8(Npi20例,NPC33例)和PCNA抗体(Npi48例,NPC80例)进行免疫组化染色。结果:(1)NPC组mIEL数量明显少于NPi组[(4.53±3.87)vs(6.33±7.20),P=0.017],但其在NPC各期和有无淋巴结转移之间变化不明显(P>0.05)。(2)NPC组mIEL与其巢内、癌巢间淋巴细胞数量(P=0.011,P=0.018)和组织中T细胞数量变化(P=0.037)均呈正相关。(3)NPC组黏膜上皮PCNA表达强度与mIEL数量变化呈正相关(P=0.007)。结论:(1)NPC组mIEL明显减少,但在其肿瘤进展过程中变化不显著;(2)NPC组mIEL变化与其组织中淋巴细胞浸润情况和T淋巴细胞变化一致;(3)NPC组mIEL数量随黏膜上皮增生活跃程度而增加。Objective:To study the changes of mucosa intraepithelial lymphocytes (mIEL) in nasopharyngeal carcinoma (NPC) and nasopharyngitis (NPi),and explore effect of local epithelial proliferation on mIEL.Methods:Paraffin embedded samples of mucosa from 51 Npi and 90 NPC cases were collected. The lymphocytes in mucosa epithelium and carcinoma nests on the HE slide were counted respectively to judge the infiltration degree of lymphoid cells. CD3,CD4 and CD8 (20 Npi cases,33 NPC cases) and PCNA (48 Npi cases,80 NPC cases)were applied with immunohistochemical stain. Results : ( 1 ) The number of mlEL in NPC group was significantly less than that in Npi group [ (4.53±3.87) vs (6.33±7.20) ] (P =0. 017), but no significant change occurred at different stages, nor between node metastasis group and group without node metastasis (P 〉 0.05). (2) The change of mIEL in NPC cases showed a positive correlation with infiltrating lymphocytes in the carcinoma nest and stroma, and with the T lymphocytes in tissues (P =0. 011 ,P =0. 018, P =0. 037). (3) The PCNA expression on mucosa epithelium in NPC group had positive correlation with the amount of mIEL ( P = 0. 007 ). Conclusion: (1) mIEL decreases more significantly in NPC group than that in NPi, but no significant change occurres during the NPC progression. (2) The change of mIEL in NPC group consists with infiltration of lymphocytes in carcinoma nest and stroma and variation of T lymphocytes in tissue. (3) There is a positive correlation between the activity of epithelium hyperplasia and the number variation of mIEL in NPC.
