摘　　要：目的:分析不同方式影像引导125I粒子植入治疗非小细胞肺癌的疗效与不良反应。方法:对确诊的晚期非小细胞肺癌155例患者制定放射治疗计划,通过CT和纤维支气管镜(FFB)影像引导植入放射性125I粒子,并行剂量学验证和定期复查随访。结果:CT引导治疗组和FFB引导组的1年生存率和有效率分别为92.2%、80.3%和97.0%、90.6%;CT组患者无放射性肺炎或肺纤维化发生;FFB组患者无早期、晚期放射损伤的发生。结论:放射性粒子植入对支气管肺癌有明显的控制疗效,具有微创的优势。CT引导植入定位准,适形好;FFB引导植入能够较好地解除气道梗阻。Objective： To analyze curative effect and damaging effect of ^25I seeds implantation brachytherapy in different way on terminal non small cell lung carcinoma （NSCLC）. Methods： Under treatment planning system and CT, FFB image guided, ^125I seeds were implanted into 155 cases confirmed terminal NSCLC and posologie validation and regular examination were done. Results： The 1-year survival rate and effective in CT and FFB image guided group were 92.2% ,80.3% and 97.0%,90.6% respectively; The cases in CT guided group had no any radioactive pneumonia or lung fiber. The cases in FFB guided group had no any radioactive injure in earlier or later period. Conclusion： The therapy of radioactive ^125I seeds implantation is resulfful and minimalivasion. ^125I implantation guided by CT has accurate location and conform shape.
