摘 要:目的观察脑钠肽(BNP)在COPD患者中合并不同心功能不全的水平变化,探讨其临床诊断价值。方法选取COPD合并左心功能不全患者10例(A组)和合并右心功能不全13例(B组),并选取同期住院不合并心功能不全的COPD患者20例作为对照组,测定它们的血浆BNP值,对比各组之间血浆BNP值的变化。结果A组、B组及C组BNP水平分别为(886.7±270.3)pg/ml、(248.3±77.1)pg/ml、(66.4±36.5)pg/ml。各组间比较,有统计学意义(P<0.01)。结论检测COPD患者血浆BNP有助于诊断患者是否合并心功能不全;血浆BNP值增高程度与心功能不全的类型有关。Objective To observe the variation of BNP level in patents with COPD and different types of heart insufficiency,and to assess its value in clinical diagnosis. Methods We recruited 42 patients in the same period after they received a diagnosis of COPD and divided them into 3 groups. Group A contained 10 patients who had COPD, combined with left heart insufficiency. Group B contained 13 patients who had COPD, combined with right heart insufficiency. Group C, as a control group, contained 20 patients who had COPD but did not have heart insufficiency. We measured BNP in plasma in these 43 patients and then analyzed the differences of its level among the 3 groups. Results There were significant differences among BNP levels in plasma of the 3 groups : ( 886. 7 ± 270. 3 ) pg/ral in Group A, (886. 7 ±270. 3) pg/ml in Group B,and(66. 4 ±36.5) pg/ml in Group C (P 〈0. 01 ). Condusion The measurement of BNP,obtained in the plasma of patients who have COPD, provides predictive information in the diagnose of heart insufficiency. The variation in the range of BNP levels are correlated with the type of heart insufficiency.
