摘 要:为了动态观察急性髓系白血病(AML)完全缓解(CR)前后的克隆性演变及其与骨髓细胞染色体核型和临床转归的关系。采用人雄激素受体(HUMARA)基因多态性──甲基化分析的方法对15例杂合子女性AML患者缓解前后的克隆性进行了分析。结果显示:15例AML患者缓解方均为单克隆造血,经化疗完全缓解(CR)后其中的11的恢复为多克隆性造血,余4例仍为单克隆性造血,后者中的一例在检查后3个月复发,未发现CR后的克隆性与骨髓三系病态造血有何关系。AML为一克隆性疾病,大部分AML患者CR后恢复正常多克隆性造血,但的有1/4的患者仍为单克隆造血。To study the clonality evolution of AML and the relationship between clonality and clinic,we analysed the clonal pattern of 15 AML before and after remission using thepolymorphism of X-linked HUMARA gene. The outcome showed that all untreated AMLpatients were monoclonal hematopoeiesis. Eleven(73% ) were polyclonal hematopoiesis in 15AML patients after remission, but the other four patients were monoclonal hematopoiesis.of four patients with monoclonal remission,one relapsed after 3 months of detection of theclonality after remission have no obvious relationship with trilineage myelodysplasia in BM.Conclusion was made that: AML is a clonal disease,and most of AML patients recover polyclonal hematopoiesis during remission,with about one-forth our patients being clonal remission.
