非痴呆型血管性认知功能障碍患者的血流动力学及血浆总磷脂水平改变

摘　　要：目的:探讨非痴呆型血管性认知功能障碍(VCIND)患者经颅多普勒超声(TCD)、血浆总磷脂(AP)水平改变及其与认知功能障碍之间的关系。方法:对52例VCIND患者、40例脑卒中不伴认知功能损害患者及42例正常对照者行脑TCD检查及血浆AP测定,使用简易智能状态检查(MMSE)量表、画钟测验(CDT)评定智能状态。结果:VCIND组AP水平显著高于正常对照组(P<0.05),VCIND组与脑卒中组之间AP水平无统计学差异(P>0.05)。与正常对照组及脑卒中组相比,VCIND组患者血流动力学异常发生率更高。VCIND组及脑卒中组患者的大脑前、中、后动脉的平均血流速度均较对照组降低;与脑卒中组相比,VCIND组大脑前动脉、大脑中动脉血流速度显著减低(P<0.05)。结论:VCIND患者存在明显的血流动力学异常和脑供血不足表现,颈内动脉系统供血不足可能与脑卒中后认知功能受损有关。Objective：To investigate the characteristics of vascular cognitive impairment non-dementia （VCIND） patients by means of transcranial Doppler （TCD）and acid phospholipids （AP）and their relationship with the cognitive impairment of these patients.Methods：Check the transcranial Doppler and AP of 52 VCIND patients and 40 stroke patients without cognitive impairment and 42 normal controls.The cognitive functions were assessed by psychometric testing（mini-mental state examination and clock drawing test） in all subjects.Results： The plasma AP levels in the VCIND group were higher than the normal contral group（P〈0.05）,but no significant difference was found between VCIND and stroke patients （P〉0.05）.Compared with the normal controls group and the stroke group,more obvious abnormal hemodynamic showed in VCIND group.Compared with the normal controls group,VCIND group and stroke group had lower blood flow velocities of anterior,middle and posterior cerebral artery,VCIND group had more obvious low blood flow velocities of anterior cerebral artery and middle cerebral artery than the stroke group （P〈0.05）.Conclusion：Obvious abnormal hemodynamic and cerebral circulation insufficiency showed in VCIND,there might be some relationship between the impaired cognitive function after stroke and carotid insufficiency.
