三维适形放疗联合肝动脉栓塞化疗治疗原发性肝癌疗效的Meta分析

摘　　要：目的评价三维适形放疗(3DCRT)联合肝动脉栓塞化疗(TACE)与单纯TACE治疗原发性肝癌的有效性和安全性。方法采用Cochrane系统评价方法,机检2009年2月以前Cochrane图书馆临床对照试验资料库、MEDLINE、CBM、CNKI以及VIP数据库;同时手检相关文献和会议论文集,按纳入标准纳入相关随机对照试验,按照Cochrane评价手册质量评价标准进行评价,用RevMan 5.0进行Meta分析。结果共纳入符合标准中文文献9篇(624例患者)。Meta分析结果显示,3DCRT联合TACE治疗原发性肝癌,在近期缓解率(CR+PR)及1、2、3年生存率均优于单纯TACE治疗(P<0.01),而在治疗后的不良反应上两者差异无统计学意义(P>0.05)。结论3DCRT联合TACE治疗原发性肝癌的疗效优于单纯TACE。Objective To evaluate the effectiveness and safety of three dimensional conformal radiotherapy （3DCRT） combined with transcatheter arterial ehemoeinbolization （TACE） in the treatment hepatocellular carcinoma, compared with TACE alone. Methods According to Cochrane criteria, MEDLINE, EMBASE, CBM, CNKI and VIP database were searched, all of the randomized control trails comparing about this object before February 2009 were enrolled. Relevant journals and conference proceedings were also hand searched. The quality of included studies was assessed according to the criteria recommended by the Cochrane Handbook 4.2.6 for Systematic Reviews of Interventions, and meta analysis were performed by RevMan 5.0 software. Results Nine randomized controlled trails, totally 624 patients were included. Meta analysis results showed that TACE plus 3DCRT,compared with TACE ＂alone, could significantly improved survival （1,2 and 3y） and CR ＋ PR（ P 〈0.01 ） ,while there was no significant difference in side effect（ P 〉0.05）. Conclusion The therapeueic effect was better with 3DCRT plus TACE than TACE in primary hepatocellular carcinoma.
