耳石手法复位治疗良性阵发性位置性眩晕的随机对照研究

摘　　要：目的:评价耳石手法复位治疗良性阵发性位置性眩晕(BPPV)的疗效。方法:将203例确诊或疑诊的BPPV患者随机分为药物组99例(其中确诊BPPV患者69例)和耳石手法复位组(CRP组,其中确诊BPPV患者73例)分别给予银杏达莫治疗(20 mL,iv,每天1次)和手法复位治疗,共14 d。于治疗第1、3、7、14天评定2组疗效,并对比分析。结果:CRP组各评定时间点疗效均明显优于药物组(P<0.001);疑诊病例首次复位疗效与药物组无明显差异,但多次复位疗效明显优于药物组(P<0.001);CRP组疑诊病例首次复位疗效差于确诊病例,但多次复位疗效与确诊病例无明显差异。结论:手法复位治疗BPPV有确切疗效,且操作简单、费用低,值得临床推广应用。Objective：To assess the therapeutic effect of canalith repositioning procedure（CRP） in the treatment of patients with benign paroxysmal positional vertigo（BPPV）.Methods： Two hundred and three patients（final or probable diagnosised as BPPV） were enrolled and randomly divided into medical treatment group（69 patients were final diagnosised as BPPV） of 99 patients who have received the Ginkgo Leaf Extract and Dipyridamole Injection（20ml iv qd）,and CRP group（73 patients were final diagnosised） of 104 patients who just accepted CRP treatment.The efficacy was assessed at days 1,3,7,and 17 after the treatments.Results： The efficacy in the CRP group was significantly better than that in the medical treatment group at all four time points（P0.001）.When compared with the patients receiving the medical treatment,the efficacy of probable BPPV patients in the first CRP treatment was not significant.However,the patients receiving repetitive CRP treatment showned a significant improvement than those in the medical treatment group at the end of the process（P0.001）.In the CRP group,the probable BPPV patients had a satisfactory improvement after repetitive CRP treatment,as patients with definite BPPV.Conclusion： The CRP treatment is an effective,simple and in-expensive treatment for the majority of patients（either final diagnosis as BPPV or probable BPPV）.
