三维适形放疗联合多西他赛同步化疗治疗中晚期食管癌的临床研究

摘　　要：目的探讨三维适形放疗（3D—CRT）联合多西他赛同步化疗在中晚期食管癌患者治疗中的疗效和毒副作用。方法96例患者根据入选标准进入研究。单药多西他赛25mg／m^2化疗，1次／wk，放疗d1开始应用。采用三维适形放疗，95％PTV66Gy／33次，共6．6wk。结果完全缓解率为67．71％，近期总有效率（CR＋PR）为89．53％。1年及3年局部控制率分别为86％、63％，1年及3年生存率分别为76.2％和49．8％。放化组毒性反应主要是放射性食管炎，其次是血液学毒性，患者均能耐受。结论三维适形放疗联合多西他赛同步化疗对食管癌近期疗效较好。虽毒性反应增加，但病人可以耐受。Objective To evaluate the effect and toxicity of three dimensional conformal radiotherapy （3D-CRT） concurrent taxotere for patients with esophageal carcinoma. Methods Ninety-six patients with esophageal carcinoma received 3D-CRT and concurrent taxotere chemotherapy. The dose of taxotere was 25 mg/m^2 once per week, and the chemotherapy was given from the first day of radiotherapy. The radiotherapy was carried out by 3D-CRT, to a total dose 95% PTV 66Gy/33f/6.6W. Results Complete response rate （CR） was 67.71%. Overall ressponse rate （CR ＋ PR） was 89.53%. The local control rates of 1 year and 3 year were 86% and 63%, respectively. The survival rates of 1 year and 3 year were 76.2% and 49.8%, respectively. The main acute toxic effect was radioactive esophagitis. Conelusion 3D-CRT concurrent taxotere, could improve the overall response rate and survival rate of esophageal carcinoma, but it increase the acute toxic effect.
