脑出血后早发性癫痫发作的发生率及其影响因素的分析

摘　　要：目的了解脑出血后早发性癫痫发作的发生率并分析其影响因素。方法采用回顾性分析病例的方法,观察脑出血后早发性癫痫发作的发生率;应用统计学方法分析脑出血后早发性癫痫发作与各影响因素的关系。结果524例脑出血患者中有早发性癫痫发作的患者37例,占7.1%,发作类型以全身强直-阵挛性发作多见。单因素分析显示,皮质出血、再发脑出血、电解质紊乱者易发生脑出血后早发性癫痫发作;Logistic回归分析显示,出血部位、出血次数和电解质情况与脑出血后早发性癫痫发作的发生有关,其中皮质出血为与脑出血后早发性癫痫发作有关的独立危险因素。结论脑出血后早发性癫痫发作的发生率较高,皮质出血、脑出血史以及电解质紊乱是影响脑出血后早发性癫痫发作的重要因素,皮质出血为独立的危险因素。Objective To investigate the incidence of early epileptic seizures after intracerebral hemorrhage and its involved factors.Methods The incidence of early epileptic seizures after intracerebral hemorrhage was observed by retrospective analysis.We also analyzed the relation of early epileptic seizures and its related factors by carrying out statistics.Results Among the 524 intracerebral hemorrhage patients,37 patients（7.1%）suffered from early epileptic seizures.The major seizure type was generalized tonic-clonic seizure.Univariate analysis revealed that a high incidence of early epileptic seizures after intracerebral hemorrhage ocurred in patients with cortical hemorrhage,the medical history of intracerebral hemorrhage,electrolyte disbalance.Logistic regression analysis revealed that the position of hemorrhage and times of hemorrhage and electrolytic states were positively correlated with the incidence of early epileptic seizures after intracerebral hemorrhage.The cortical hemorrhage was the independent risk factor for the early epileptic seizures after intracerebral hemorrhage.Conclusions The incidence of early epileptic seizures after intracerebral hemorrhage was high.The cortical hemorrhage and the medical history of intracerebral hemorrhage,electrolyte disbalance were the important related factors for the incidence of early epileptic seizures after intracerebral hemorrhage.The cortical hemorrhage was the independent risk factor.
