正畸治疗中下颌第一前磨牙拔除后下颌牙自然移动的临床研究

摘　　要：目的:观察不同类型错下颌第一前磨牙拔除后下颌牙的自然调整变化。方法:20例因正畸需要拔除下颌第一前磨牙的病例为研究对象,其中安氏Ⅰ类错和安氏Ⅱ类错各10例。第一前磨牙拔除后6个月内下颌先不进行矫治,上颌行MBT矫治技术娇治。分别测量两组病例拔牙后30min、4个月时拔牙间隙近、远中方向最小径,比较分析两者拔牙间隙变化情况和Spee's曲线曲度。结果:拔牙4个月后与拔牙后30min比较,两组拔牙间隙最小近远中径减小了1.35mm和1.26mm(P<0.01),Spee's曲线曲度也有显著性改变,分别减小了0.28～0.32mm,差异有统计学意义。结论:MBT技术治疗安氏Ⅰ类和安氏Ⅱ类拥挤错,可以通过下颌第一前磨牙拔除后下颌牙自行调整,减小拔牙间隙,改善Spee's曲度。Objective： To study the physiological drift of the mandibular teeth after extraction of the first mandibular premolar. Method： 20 patients with the needs of the first mandibular premolar＇s extraction were applying MBT appliance to the upper teeth first and no appliance on the mandibular teeth in 6 month. The diameters of the post extraction spaces and curvature of spee＇s curve measured at the time of 4 months after the first premolar＇s extraction were compared with that measured at the time of 30 minutes after the first premolar＇s extraction. Result： There were significant differences in the diameters of the post extraction spaces and curvature of spee＇s curve measured at the time of 4 months and of 30 minutes after the first premolar＇s extraction. Conclusion： Angle I and Angle Ⅱ crowed malocclusion could be successfully treated with first premolar extraction, applying M BT appliance to the upper teeth first, by drift of the mandibular teeth.
