乳腺导管造影术在乳腺导管内癌中的应用

摘　　要：目的:评价乳腺导管造影对有乳头溢液导管内癌的诊断价值。方法:回顾性分析28例有乳头溢液的导管内癌资料,病例均行钼靶X线平片摄影、乳腺导管造影检查及手术病理确诊。结果:乳腺钼靶X线平片有阳性征象者7例,占25.0%,乳腺导管造影明确诊断25例,占89.3%。造影主要表现为大导管及分支导管管壁不规则浸润、虫蚀样破坏、僵硬及狭窄,腔内多发小充盈缺损,近端导管扩张,主导管及部分分支中断。结论:乳腺导管造影对乳头溢液的导管内癌诊断具有重要价值。Objective To evaluate the value of galactography in the diagnosis of ductal carcinoma in situ of breast with nipple discharge.Methods Twenty-eight patients with ductal carcinoma in situ with nipple discharge were analyzed retrospectively.All cases were proved by molybdenum target manmography,galactography and pathology.Results Mammogram showed positive sign in 7 cases（25.0%）.Twenty-five （89.3%） were diagnosed correctly by galactography.The characteristic image of ductal carcinoma in situ in the mammary ductography included irregular invasion,moth-eaten destroy,rigidity and narrowing,multiple intraluminal filling defect,near portion ductal dilation,interruption in the main and branch ducts.Conclusion Galactography is of great value to the diagnosis of ductal carcinoma in situ of breast with nipple discharge.
