乳头糜烂性腺瘤病1例

摘　　要：患者女,50岁。左乳头反复糜烂、结痂1年。皮肤科情况:左乳头皮肤略隆起,中央有角化痂,去除角化痂皮后,露出红色糜烂面。皮损组织病理示:肿瘤位于真皮内,与表皮相连,无包膜,可见乳腺导管上皮细胞明显增生,形成大小不等的管样结构和实体细胞巢,周围可见肌上皮细胞包绕,间质内透明样变性及少量淋巴细胞及浆细胞浸润。免疫组化:肿瘤团块E-cadherin强阳性。行局部乳头及乳晕楔形切除术,现随访中。A 50-year-old female presented with one year history of keratosic crust and erosive erythema repeatedly in the left nipple . Skin lesions were keratosic crust and erosive erythema after removing keratosic crust. Skin biopsy showed that tumor connected epidermis without envelope and epithelial luminal cells proliferated to form inequality of size lumens or cell nests predominantly in the dermis; Basalmyoepithelial cell layer was intact. There were several mucinous degenerations and small amounts of lymphocytes and plasmocytes in stroma. Immunohistochemical analysis showed that E-cadherin was strongly positive. A histolotical diagnosis of erosive adenomatosis of the nipple was given. Part of left nipple and the skin around it was cut and the patient was followed at present.
