摘　　要：目的观察胰岛素样生长因子Ⅱ(IGF-Ⅱ)及B细胞淋巴瘤因子-2(Bcl-2)在肝细胞癌变过程中的动态表达及变化特点。方法以2-乙酰氨基芴(2-FAA)喂饲雄性SD鼠诱发肝癌发生,分析肝细胞病理形态学改变,以免疫组织化学法观察鼠肝细胞IGF-Ⅱ和Bcl-2的表达与定位,并定量分析肝IGF-Ⅱ和Bcl-2蛋白水平变化。结果诱癌过程中肝细胞出现颗粒样变性、不典型增生到HCC的形成过程中IGF-Ⅱ呈梯度表达,表现为癌变组明显高于对照组和变性组(P<0.01),癌变过程中IGF-Ⅱ和Bcl-2呈阳性表达和胞内分布,肝IGF-Ⅱ与Bcl-2呈显著正相关,癌变组明显高于变性组和正常对照组(P<0.01)。结论IGF-Ⅱ异常活化和Bcl-2过表达均参与肝细胞的癌变过程,对它们的分析有助于肝癌的早期诊断和肝癌发展的监测。Objective To investigate the dynamic expression and alteration characteristic of insulin-like growth factor-II （IGF- II ） and B-cell lymphomas-2 （Bcl-2） during development of hepatocellular carcinoma （HCC）. Methods Rat hepatoma models were made with 2-fluorenylacetamide （2-FAA） fed with male Sprague-Dawley （SD） rats. Morphological changes of rat livers were observed during HCC development. The expression and distribution of liver IGF- II and Bcl-2 were analyzed by immunohisto-chemistry, and the dynamic changes of them in liver were quantitatively analyzed. Results The levels of hepatic IGF-II gradually increases in the process of rat hepatocytes from granule-like degeneration to atypical hyperplasia to HCC. The level of IGF-II in hepatoma was significantly higher than any of other groups. The positive relationship of IGF-II was found in liver （P 〈0.01 ）. Bcl-2 and IGF-II levels in hepatoma were significantly higher than those in other groups （P 〈0.01 ）. Conclusion Hepatic IGF-II and Bcl-2 may participate in hepatocyte canceration and the over-expression could be applied in the early diagnosis and monitoring of HCC.
