摘 要:目的研究HIF-1α和P-gp蛋白在胰腺癌组织中的表达并探讨其临床病理学意义及两者之间的相关性。方法采用免疫组织化学(SP)法检测58例胰腺癌组织、12例慢性胰腺炎组织和10例正常胰腺组织中HIF-1α和P-gp蛋白的表达。结果HIF-1α的阳性表达主要在细胞核或细胞浆内,呈棕黄色颗粒,P-gp则表现为在细胞膜或细胞浆内出现棕黄色颗粒。HIF-1α和P-gp在胰腺癌组织中的阳性表达率分别为68.97%(40/58)和58.62%(34/58),显著高于两者在正常胰腺组织和慢性胰腺炎组织中的表达,差异有统计学意义(P<0.0125)。HIF-1α和P-gp的异常表达均与胰腺癌的淋巴结转移和TNM分期相关(P<0.05),与患者的性别、年龄、肿瘤部位、分化程度和病理分型无关(P>0.05)。HIF-1α和P-gp蛋白的表达之间存在正相关性(r=0.671,P=0.031)。结论胰腺癌组织中存在HIF-1α和P-gp蛋白表达的上调,并且它们的表达存在正相关性。联合检测两个指标对临床化疗疗效判断和患者预后估计具有积极的指导作用。Objective To investigate the expression and clinical pathological significance of HIF-1α and P-gp protein in human pancreatic carcinoma and to explore the correlation between HIF-1α and P-gp protein. Methods SP (streptavidin-peroxidase) immunohistochemical method was used to detect the expressions of HIF-1α and P-gp in 10 cases of normal pancreas, 12 cases of chronic pancreatitis and 58 cases of pancreatic carcinoma specimens separately. Results The positive staining of HIF-1α exhibited brown-yellow granules in the cytoplasms and/or cell nuclei, whereas P-gp was mainly in cytoplasm and with some staining on cell membranes. The positive expression rates of HIF-1α and P-gp were 68.97% (40/58) and 58.62% (34/58) respectively in pancreatic carcinoma tissues, both of which were significantly higher than those in normal pancreas tissues and chronic pancreatitis tissues ( P 〈 0. 0125 ). The expressions of HIF-1α and P-gp in pancreatic carcinoma showed an obvious correlation with lymph node metastasis and TNM stages (P 〈 0.05, respectively) , but there was no correlation with the patient' s gender, age and location, differentiation degree, pathology, typing of tumors (P 〉 0.05). Significant correlation existed between the expressions of P-gp and HIF-1α in pancreatic carcinoma ( r = O. 671 , P = 0. 031 ). Conclusion Up-regulated expressions of HIF-1α and P-gp are ohserved in pancreatic carcinoma and they are significantly correlated. Combined detection of the two indicators can help with accurate evaluation of the clinical effects of chemotherapy and may guide the estimation of the prognosis in patients with pancreatic cancers.
