微小胃肠道外胃肠道间质瘤的诊断及鉴别诊断

摘　　要：目的探讨外科淋巴结清扫中对意外发现微小胃肠道间质瘤(GIST)的病理诊断及鉴别诊断。方法对3例外科淋巴结清扫标本中发现的微小GIST进行光镜及免疫组化观察。结果镜下瘤细胞为梭形细胞,异型性小,成束状或编织状排列,核分裂像偶见;免疫组化显示瘤细胞CD117(+++),CD34(+++),S-100(-),Des(-),SMA(-)。结论在外科淋巴结清扫标本中,对发现的微小梭形细胞肿瘤要高度警惕GIST的可能,并通过光镜观察及免疫组化检测可进行诊断及鉴别诊断。Objective To discus surgical lymph node dissection of the accident found that small gastrointestinal stromal tumors of the pathological diagnosis and differential diagnosis.Methods Three cases of lymph node dissection specimens of small GIST was found by light microscopy and immunohistochemical observation.Results Microscope,the tumor cells to spindle cells,small atypia,or woven into a bundle-like arrangement,like the occasional mitotic;immunohistochemical results showed that the tumor cells CD117 （＋＋＋）,CD34 （＋＋＋）,S-100 （-）,Des （-）,SMA （-）.Conclusion Lymph node dissection in surgical specimens of small spindle cells found in the tumor to be highly vigilant against the possibility of GIST,and through light microscopy and immunohistochemical detection for diagnosis and differential diagnosis.
