老年食管疾病的胃镜诊断分析

摘　　要：目的探讨老年食管疾病内镜检出率和临床特点。方法以非老年组作为对照，回顾性分析我院内镜中心2005年～2006年480例老年食管疾病的检出率和临床特点。结果老年食管疾病的检出率明显高于非老年组（P〈0．05），依次为食管炎（17．8％）、食管癌（8．1％）、食管溃疡（3．2％）、食管裂孔疝（1．5％）、食管静脉曲张（1．1％）、食管息肉（0．8％）和其他（0．9％）；烧心、反酸、胸骨后疼痛或不适为其主要临床表现。结论老年食管疾病呈多样性，以食管炎、食管癌和食管溃疡为多见，胃镜检查有助于诊断。Objective To explore the endoscopic detection rate and clinical characteristics of esophageal diseases in elderly people. Methods Four hundred and eighty elderly cases of esophageal diseases at the Endoscopy Center from 2005 to 2006, were analyzed retrospectively on the endoscopic detection rate and clinical characteristics, and compared with non-elderly cases. Results The endoscopic detection rates of esophageal diseases in the elderly were much higher than in the non - elderly. The esophageal diseases included esophagitis （ 17. 8% ）, esophageal carcinoma （ 8. 1% ） , oesophageal ulcer （3.2%）, esophageal hiatal hernia （1.5%）, esophageal varices （1.1%）, esophageal polyps （0.8%） and the others （0.8%）. Heartburn, acid reflux, and retrosternal pain or discomfort were the main clinical manifestations. Conclusion The esophageal diseases in the elderly shows diversity, the most common of which are esophagitis, esophageal carcinoma and esophageal ulcer. Endoscopic examination is helpful in the diagnosis of esophageal diseases in the elderly.
