老年人白内障超声乳化术后低视力临床分析

摘　　要：目的分析白内障超声乳化吸出联合人工晶状体植入术后低视力的相关因素及解决对策。方法收集资料完整的老年性白内障患者982例(982眼),对其行超声乳化吸出联合人工晶状体植入术,术前矫正视力光感～0.4。0.3>最佳矫正视力≥0.05者为低视力诊断标准,对其发生原因进行回顾分析。结果982眼患者术后低视力共98眼,其中手术所致术后低视力35眼,术前病变所致术后低视力63眼。手术并发症致低视力的原因多为角膜水肿,术前病变致低视力的原因主要是糖尿病视网膜病变、年龄相关性黄斑病变、高度近视黄斑病变等。结论老年性白内障超声乳化吸出联合人工晶状体植入术后影响视力提高的主要原因为术前存在的眼底病变。减少术中及术后并发症及充分估计导致术后低视力的危险因素是避免术后低视力的关键。Objective To analyze the main causes and strategies of low vision after phacoemulsification and intraocular lens（IOL）implantation.Methods The complete data of 982 senile cataract patients（982 eyes）,who were treated with phocoemulsification and IOL implantation,were collected.The preoperative corrected vision was light perception to 0.4.Best corrected visual acuity be and more than 0.05 and less than 0.3 was the diagnostic criteria for low vision.The causes of low vision were retrospectively analyzed.Results In 982 eyes, there were 98 eyes with postoperative low vision,35 eyes caused by operation and 53 eyes by preoperative pathological changes. Corneal edema was the main complication of operation to cause low vision. Diabetic retionpathy, age-related macular degeneration and high myopia maculopathy were the main preoperative pathological changes for low vision. Conclusion Preoperative pathological changes at ocular fundus is the main reason to influence the improvement of vision after phacoemulsification and IOL implantaion in senile cataract patients. The key points for avoiding the incidence of low Vision are reducing the intraoperative and postoperative complications and fully estimate the risk factors causing postoperative low vision.
