摘 要:目的应用fMRI技术,观察左侧颞叶内侧癫痫(MTLE)患者语言区的优势侧别。方法对8名右利手左侧MTLE患者和15名右利手健康志愿者,进行听觉词汇语义判断任务,采集脑功能图像。数据分析采用功能神经成像分析软件(analysis of functional neuroimage,AFNI)进行处理。根据脑功能活动激活结果,计算两侧脑区在语言认知任务上的定侧指数(laterality index,LI)。结果组分析结果显示:MTLE组Broca区和Wernicke区LI平均值分别为-0.29和-0.25,都为右侧半球优势。个体分析显示:MTLE组Broca区6例(75.0%)为不典型语言优势(右侧及双侧);Wernicke区5例(62.5%)为右侧优势。与正常对照组比较,MTLE组Broca区、Wernicke区不典型语言优势例数有显著性差异。结论对左侧MTLE患者而言,语言区的优势半球评价,fMRI是有价值的技术。Objective To lateralize hemispheric language dominance in left medial temporal lobe epilepsy(MTLE) using fMRI. Methods Eight patients with left MTLE were evaluated. Auditory semantic judgment (AJ) paradigm was used. The fMRI data were collected on a 3T MR system and analyzed by AFNI (analysis of functional neuroimages). On the basis of the generated activation map, hemispheric dominance for language was calculated in the Broca's and Wernicke's regions using laterality indices (LI). Results The group analysis showed that the mean LI were -0. 29 and -0. 25 resprctively in left MTIJE patients, which indicated right hemisphere dominance. The individual analysis showed that atypical language representation in Broca's region was found in 75% of left MTLF patients and right language representation in Wernieke's region was found in 62.5% of left MTLE patients. Conclusion fMRI is a valuable technique in lateralizing hemispheric language dominance in left MTLE.
