摘 要:目的测定浙江省杭州、绍兴、诸暨、舟山四地区正常青少年肺功能正常值及探讨不同生活环境、年龄、身高、体重等因素与肺功能的关系。方法采用便携式肺功能仪对777名健康青少年(男性418人、女性359人)进行肺功能检测,分析不同地域青少年肺功能状况及其变化趋势,取最能反映肺功能的四个指标(FVC、FEV1、PEF、MMEF75/25)做多元逐步线性回归以检测地域、年龄、性别、身高、体重等因素对肺功能的影响。结果大部分肺功能指标男性大于女性,男性肺功能以绍兴地区最强,女性肺功能以舟山地区肺功能最强,杭州地区男、女性肺功能均最弱;男性肺功能随年龄增长而增长至26岁时仍有增长趋势,女性肺功能在16岁左右达到峰值,其后变化呈波动趋势;多元线性回归结果显示FVC、FEV1、PEF、MMEF75/25与身高、年龄、体重均正相关,但身高对肺功能的影响持续存在,而体重和年龄对肺功能的影响则因指标而异。结论本研究首次建立了浙江省杭州、绍兴、诸暨、舟山四地区正常青少年肺功能正常值及其回归方程,肺功能生理预计值存在地域差别,多数肺功能指标随年龄的增长而增长,地域、身高、年龄对肺功能的影响不容忽视。Objective To put up the spot test and investigate the lung function of normal adolescents living in Hangzhou City, Shaoxing City, Zhuji area, and Zoushan area in Zhejiang Province and its correlative factors such as living circumstances, age, height, and body weight. Methods The lung volumes and breath capacities of 777 normal adolescents, including 418 boys and 3.59 girls were tested by portable spirometers, the status and the changes of their lung functions were analyzed. We carried out multiple linear regression analysis on four indicators (FVC, FEV1, PEF, and MMEF75/25) to evaluate the effect of the factors, including region, age, sex, height, and weight on lung function. Results Significant difference existed in some indexes among urban, rural and fishery areas (P〈 0. 05, P〈 0. 01 ). The value of male indexes was more than that of female indexes; the value of normal male adolescents of Shaoxing City and the value of normal female adolescents of Zoushan Area were significantly higher than these of adolescents in other areas. The value of adolescents of Hangzhou City was significantly lower than those of people in other areas. Growth spurt of female subjects appeared at the age of 16 and then uncertainly changed by age average, the growth trend of male subjects' value was maintained on the age of 26. The results of multiple linear regression analysis showed that FVC, FEV1, PEF, and MMEF75/25 were positively correlated with height, age, and weight. Among these factors, height had the most influential effect while age and weight took the second place. Conclusions This study provides reference values of lung function and regression equation for normal adolescents in Hangzhou City, Shaoxing City, Zhuji area, and Zoushan area of Zhejiang Province. Expected values of lung function are changed regionally because of different nationalities and living conditions. Most of the indicators of lung function change with the increment of age and are affected by region and height.
