放化疗结合相控阵聚焦热疗治疗直肠癌术后局部区域复发的临床观察

摘　　要：目的:探讨放化疗与相控阵聚焦热疗同步治疗直肠癌术后复发的疗效及安全性,并与单纯放化疗进行比较。方法:将直肠癌术后复发患者随机分为热放化组(治疗组)及单纯放化组(对照组),其中冶疗组患者采用三维适形放疗,2.5～4Gy/(次.d),5次/周。总剂量DT48～65Gy,并口服卡培他滨2500mg/(m2.d),连用14d。同时配合应用相控阵聚焦热疗系统,2次/周,时间为45～60min/次,直到治疗结束。对照组放化疗方法同治疗组。结果:治疗组中完全缓解(CR)占20.5%,部分缓解(PR)占64.7%,有效率为85.3%,对照组完全缓解(CR)占16.1%,部分缓解(PR)占41.9%,有效率为58.1%;疼痛症状缓解率治疗组为92%,对照组为72.7%,两组比较差异有统计学意义,P<0.05;毒副反应发生率两组比较差异均无统计学意义,P<0.05。结论:放化疗与相控阵聚焦热疗同步治疗直肠癌术后复发疗效及症状改善方面明显提高,且毒副反应无明显增加。OBJECTIVE：To investigate the efficacy and side-effect of focus phased array thermotherapy plus radiotherapy and chemotherapy for rectal cancer with recurrence after radical anterior resection.METHODS：Sixty-five patients were divided into two groups randomly.The patients in the treatment group were treated by focus phased array thermotherapy plus radiotherapy and chemotherapy with the total dose of 48-65 Gy,2.5-4 Gy/d,five times/week,and capecitabine,fourteen days,2 500 mg/（m2·d）,in the meantime receiving thermotherapy twice per week,45-60 minutes every time.The patients in the control group received radiotherapy and chemotherapy.RESULTS：In the treatment group,the rate of complete remission（CR）was 20.5%,partial remission（PR）was 64.7%,and the overall response rate（CR＋PR）was 85.3%;while in the control group（92%）,CR and PR were 16.1% and 41.9% respectively,and the overall response rate（CR＋PR）was 58.1%.The overall response rate in the treatment group was significant higher than that of the control group（72.7%,P〈0.05）.No significant difference was found in toxicities between the two groups.CONCLUSION：Radiotherapy and chemotherapy combined with focus phased array thermotherapy is efficient and safe for rectal cancer with recurrence after resection.
